Job summary
37.5 hours per week - permanent.
The CNRT deliver patient-centred MDT neuro-rehabilitation to patients with neurological diagnosis across Plymouth, South Hams and West Devon. We are driven to help patients maintain their independence and quality of life by delivery patient goal-based rehabilitation programmes.
You will contribute in the delivery of rehabilitation under guidance from registered professionals to support patients achievement. This will be achieved through implementing a variety of therapeutic interventions and practise of activity of daily living depending on patient's needs.
You will provide support and supervision in all aspects of rehabilitation to patients. Rehab is delivered in a variety of settings including CNRT patient homes, outpatients and group classes at Mt. Gould Hospital.

Main duties of the job
Work as an outpatient and community-based rehabilitation assistant and undertake an active role in supporting planning, co-ordination, delivery, development and evaluation of the Community Neuro Rehabilitation Service.
Follow care plans from registered staff for occupational therapy, physiotherapy, speech and language therapy and nursing interventions to support the rehabilitation of patients.

Job responsibilities
The post-holder will provide support in all aspects of multidisciplinary assessment and treatment of patients, facilitating a wide variety of therapeutic interventions and activities depending on service needs (this may include, one to one, group or joint working with colleagues).
Provide support and supervision to patients to enable access and participation in outpatient, home and community-based settings.
The post holder will work with all patients referred into the service. Support the running of the department including administrative responsibilities and infection control measures.

- Clear evidence of understanding of neuro rehabilitation process and services.
- Clear evidence of understanding the roles multi-disciplinary team.

Experience

Qualifications

- Recognised level 3 qualifications or demonstrated ability and commitment to achieve a relevant level 3 qualification.
- Level 2 functional skills, or as a level 3 apprenticeship such as the
- Senior Healthcare Support Worker apprenticeship (or other level 3 qualification pathway).
