Job summary
As a Registered Nursing Associate at Stonewood, you will support the delivery of safe, effective, person-centred care, working alongside Registered Nurses and the wider multidisciplinary team. You will contribute to clinical care, support carers in the community, and help maintain the highest standards of quality, safety and governance.
Main duties of the job
Key responsibilities include:
- Supporting the development and review of personalised care plans, medication records, and clinical documentation under the direction of a Registered Nurse.
- Carrying out routine clinical observations, contributing to care reviews, and supporting initial client assessments.
- Recording incidents, monitoring clinical concerns through Birdie, and escalating risks promptly and appropriately.
- Delivering high quality, person-centred care that promotes clients' dignity, independence and wellbeing.
- Building positive relationships with clients, families and carers while providing day-to-day clinical support and guidance.
- Holding a valid UK Nursing Associate NMC registration, previous Primary or Secondary Care experience, and a full UK driving licence with access to your own vehicle.

Job responsibilities
Role Summary
As a Nursing Associate, you will support the Registered Nurses in delivering safe and effective care within their defined scope of practice. You will contribute to the implementation and monitoring of care plans, provide hands on clinical support, carry out routine clinicals tasks and escalate concerns appropriately.
You will work collaboratively as part of the wider multidisciplinary team (MDT) to ensure coordinated and high-quality care delivery. You will also help maintain clinical standards, promote client wellbeing and contribute to responsive and supportive care environment, with
participation in the on-call rota.
Main Duties and ResponsibilitiesCare Planning & Clinical Oversight:
- Contribute to the development, implementation and updating of personalised care plans based on client needs and observations, under the direction of a Registered Nurse
- Support updates to medication records in line with prescribed changes, ensuring accuracy and timely documentation
- Undertake routine clinical observations and participate in care reviews to support quality and safely monitoring
- Support the initial assessment process for new clients by gathering and recording relevant clinical information as directed by the Registered Nurse
- Accurately document incidents, observations and care delivery and contribute to follow up discussions and learning reviews
- Monitor and review Birdie concerns and inbox items, escalating any clinical issues or risks promptly to the Registered Nurse or Head of Clinical Delivery
Learning and Development:
- Support the delivery of on-site and simulation-based training to carers in areas such as medication administration, moving and handling and other routine clinical care tasks under the direction of a Registered Nurse or Head of Clinical Delivery
Assist in supporting new carers to achieve the Care Certificate and agreed clinical competencies through demonstration, guidance and feedback
Participate in internal and external training to maintain and develop clinical skills and professional competence
Contribute to competency assessments and development discussions, escalating any concerns regarding staff performance or competency to the Registered Nurse or Head of Clinical Delivery
Support clinical audit processes and quality improvement activities by providing accurate feedback from frontline care delivery
Client and Carer support:
- Provide day to day care and support to clients in line with individual care plans, promoting dignity, independence and person-centred care
- Build and maintain positive, professional and supportive relationships with clients and their families
- Support carers working in the field by sharing observations and escalating concerns or clinical changes to the Registered Nurse or Head of Clinical Delivery as appropriate
- Contribute to a responsive, person centred culture of care by accurately reporting changes in client condition, wellbeing or risk
- Participate in home visits, reviews and meet and greet visits as part of the wider care team, supporting information gathering and continuity of care
- Contribute feedback on carer performance, wellbeing and development needs within supervision or appraisal processes, escalating concerns where necessary
- Support introductory meetings with new or prospective staff to assist onboarding and understanding of care delivery expectations
Liaison and Professional Collaboration:
Work collaboratively with the Head of Clinical Delivery and wider office and care teams to support safe and effective care delivery
Communicate and liaise with Registered Nurses, GPs, district nurses, hospitals and other members of the multidisciplinary team (MDT) to support coordinated care planning and delivery
Participate in multidisciplinary meetings where required, contributing observations and updates to support client care decisions
Escalate any safeguarding concerns immediately to the Registered Nurse
Maintain active NMC registration, adhere to the NMC code of conduct and take responsibility for ongoing learning and professional
participate in training to maintain competency
On call responsibilities:
- Participate in the clinical on-call rota as required, providing support to the on call Registered Nurse and care team during out-of-hours period
- Support the escalation of concerns by gathering information, monitoring client status and communicating updates to the Registered Nurse or Head of Clinical delivery
- Assist in the management of urgent situations by following direction from the Registered Nurse and ensuring appropriate actions are carried out in line with care plans and organisational procedures
Experience, skills and knowledge required for the role
- Registered Nursing Associate with valid NMC registration, with no restrictions on practice.
- Understanding of clinical care delivery within a delegated and supervised framework
- Previous experience in Primary or Secondary Care.
- Ability to deliver safe, person centred care and work effectively as part of a team under the direction of the Registered Nurses
- Excellent communication and problem-solving skills.
- Ability to work under pressure, prioritise workloads and meet tight deadlines
- Confidence with digital care systems (for example, Birdie) and MS Office
- Full UK Driving Licence and access to own transport.
- Participation in the on-call rota.
