Job summary

Wymondham Medical Partnership are currently looking to expand our Outreach team with the creation of a new Healthcare Assistant role. The role would involve visiting patients homes to complete observations and phlebotomy duties.

Use of own car essential.

Main duties of the job

Health Checks and assist with health promotion incentives.

To undertake and record baseline observations i.e., BP, pulse, oxygen saturation, height, and weight.

To perform venepuncture for patients.

To undertake urinalysis using appropriate equipment, highlight any abnormalities and record on clinical system.

Routine and 24-hour BP monitoring.

Ear assessment - following appropriate training.

Assist with specific long term condition reviews e.g., COPD, diabetes, hypertension, learning disabilities, SMI checks.

To undertake assessment of wounds/ dressings following appropriate training.

To assist with NHS seasonal health campaigns, such as flu vaccinations.

Administer flu and Pneumonia injections, as per patient specific directives following specific training.

To assist in the early identification of dementia/ adult and child safeguarding concerns and highlight to relevant health professionals.

Following training to perform other clinical duties to meet the needs of the practice.

Refer patients to an alternative care setting or treat and discharge as appropriate.

Participate in identification of community health needs and develop patient / family centred strategies to address them.

To provide support to the surgery team to function effectively.

To perform other task within the skills of the post holder.

About us

Wymondham Medical Partnership is large single site GP surgery based in Wymondham Norfolk. We have a patient list size of nearly 20,000.

We have a team of over 70 staff including GPs, ANPs, Practice Nurses, Physician Associates, Healthcare Assistants and Clinical Pharmacists.

Working alongside the General Practice Nurses and strictly in accordance with specific practice guidelines and protocols, the health care assistant will

Assist all colleagues in the care of practice patients to include treatment, preventative care, screening, and patient education. The post will include some administrative duties.

Act as a focal point of communication between patients, doctors, nurses, and other members of the primary health care team. To actively participate in health promotion for patients and their families

Work collaboratively with the multi-disciplinary general practice team to meet the needs of the patients.

Confidentiality:

As per both Government legislation and Practice policies ensure that all confidentiality, data protection and information governance policies and guidelines are followed and strictly adhered to, reporting any infringements to the Nurse Manger or Practice Manager immediately.

Administrative and Professional Responsibility:

Participate in the administrative and professional responsibilities of the practice team.

Ensue the clinical computer system is kept up to date with accurate details recorded at the time of consultation.

Attend and participate in practice meetings as required.

Ensure total familiarity with the computer systems used within the surgery i.e., SystemOne, teams.

Book appointments and recalls ensuring sufficient information is recorded to enable retrieval of the medical record.

Personal and professional development:

The post holder will participate in any training programme implemented by the practice as part of this employment.

Participate in annual appraisals and 1:1.

Maintain a record of their own personal and professional development.

Take responsibility for their own learning, development and performance demonstrating skills and activities to others who are undertaking similar work.

Contribution to the Implementation of Services:

Discuss with other members of the team how the policies, standards and guidelines will

affect own work.

Participate in audit where appropriate.

Actively take part in mandatory and internalexternal training as requested.

Be aware of government guidelines in relation to confidentiality, clinical information etc.

Adhere to and follow all policies and protocols as listed in the Staff Handbook and contract and apply practice policies, standards, and guidance.

Follow all Practice Health and Safety guidelines and policies and inform the H&S lead in the surgery of any infringements.

Qualifications

Essential

  • 3 GCSEs at Grade A-C or equivalent

Desirable

  • Care Certificate
  • Phlebotomy qualified
