Job responsibilities

1. Job Purpose and scope

Provide clinical leadership and management support to thePall 24 team, ensuring the development and delivery of a responsive, evidence-based,high qualityend of life care service.

Work with lead nurses and other senior staff to promote an integrated and cohesive team aimed at improving care and patient outcomes.

Responsible and accountable for ensuring a safe,effectiveand transparent service; including aspects of Health & Safety, Infection prevention and control, risk management and creating a culture of open discussion.

Work with the Director ofNursing and ClinicalServices and the Medical Director to shape our vision for the future ofthe Pall 24 service, engaging with patients, carers, key local stakeholders in health and social care to improve patient care, promote the role of the hospice, and ensurethe Pall 24 serviceremains central to the delivery of good end of life care.

Contribute to the wider strategic development of the hospice and help to translate local and national strategy relating to community services, enabling us to implement and deliver an effective service that reflects and is responsive to our current and future needs.

Deputisefor the Director ofNursing and ClinicalServices on specific areas of clinical practice and strategic development relating to your leadership remit. In the absence of the Director ofNursing and ClinicalServices,work with leadership peers to respond to enquiries, manage servicesand support the staff asrequired.

Working with the other clinical services leadspromotingan integrated patient service wide culture, which encourages seamless and cross department working and cover for team leads colleagues in their absence.

Participate in the clinical on-callrota.

1. Main Duties & Responsibilities

Professional and Clinical:

• Be visible,accessibleand professional in your leadership and management of the service.

• Work with thePall 24 teamand other senior colleagues toensurethe delivery of a high quality, patient-centredservice that isevidence basedand responsive.

• Represent the hospice at meetings and other public forumsasrequired.

• Promote the hospice and increasedawareness of the range of services provided,maximisingthe number and appropriateness of referrals.

• Work withservice leadand other hospice colleagues to ensure there is a clearly defined pathway for referrals to our services.

• Liaise with key professionals andpartnerorganisationsto ensure thePall 24 serviceisdeveloped to be responsive and to support the delivery of an integrated, patient-focused service.

• Maintain clinical skills and knowledge through clinical decision making and supervising nursing practice

• Promote awareness of safeguarding adults and children, highlighting any concerns to the Safeguarding lead for the Hospice.

• Be up-to-date and aware of current policy, innovations andevidence basedpractice, responding to new guidance and policy appropriately.

• Lead and support the team to openly discuss and challenge standards of individual practice, using reflection and critical incidents as a learning opportunity.

• Ensure that the dignity,safetyand confidentiality of patients arerespected at all times, and that all patients receive the highest possible standards ofcare.

• Promoteand role modelstandards ofbehaviourand professionalismwithin the Pall 24 team.

Managerial

• • The post holder willdeputisefor the Director ofNursing andClinical Services on specific areas of clinical practice and strategic development relating to your leadership remit. In the absence of the Director ofNursing andClinicalServices,work with leadership peers to respond to enquiries, manage service and support the staff asrequired.

  • Work as a member of the senior clinicalteam to develop operational plans for improving and developing effective and efficient clinical services for allend-of-lifepatients and families/carers and the health and social care professionals who work with them.

  • Work with the Director ofNursing and ClinicalServices and Medical Director to shape the vision and strategy forthe Pall 24 service, leading on negotiations with externalorganisationsasrequired.

  • Work with thePall 24 teamto develop an annual work program, actionplanand review.

  • Undertake such other duties and responsibilities as may bereasonably required, consistent with the scope and responsibilities of the role and clinical remit, as delegated by the Director of Nursing and Clinical Services.

  • Work with the Director ofNursing and ClinicalServices andthe finance teamto effectively manage and proactively set budgets for thePall 24team.

  • Work with the Director ofNursing and Clinical Servicesandthe fundraising teamto support the development of charitable bids and fundraising projects.

  • Be responsible foradequate and safestaffing of the servicebymaintainingadutyrotaand monitoring absences and leave arrangements including the use of bank and agency staff.

  • Work with Service Leads, People andOrganisationalDevelopment Department and Director of Nursing and Clinical Services to ensureappropriate advertising,selection, appointment and orientation ofnew staffand volunteers
