An exciting opportunity has arisen within our organisation and we are seeking an experienced and motivated Paramedic Practitioner to join our friendly and supportive General Practice team. This is an exciting opportunity to work at the top of your licence within primary care, delivering high quality, patient centred care.

The post is suitable for a registered Paramedic with an Independent Prescribing qualification, or someone actively working towards prescribing and supported to complete it.

Main duties of the job

  • Assess, diagnose, and manage patients with acute and chronic conditions
  • Undertake same day and routine appointments
  • Manage minor illness, long term conditions, and urgent presentations
  • Prescribe safely and appropriately (where qualified)
  • Order and interpret investigations
  • Provide clinical advice and support to the wider MDT
  • Contribute to service development and quality improvement initiatives
  • Maintain accurate, high quality clinical records
  • Assist with Anima Total Triage
  • The post holder, acting within their own professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the Practice within the clinic setting and undertaking home visits for patients who fall within their clinical remit.

    You will be supported daily by a supervising GP who you will have constant contact with as required.

    Marden Medical Centre is a focal point for the local community and was rated as outstanding in our most recent CQC inspection. We provide services to 7,700 patients under a general medical services contract (GMS) including an on-site dispensary.

    We work closely with our Patient Participation Group (PPG) and the registered charity, Friends of Marden Medical Centre who both make a valuable contribution to the Medical Centre and the wider community we serve. We are also a part of the Weald Primary Care Network (WPCN) and more widely West Kent Primary Care where one of our GP Partners is currently Vice-Chair. Our team have a particular focus on delivery learning and development for the PCN.

    Learning and improving is central to the way we work at Marden Medical Centre. We are proud to be an accredited General Practitioner Training Practice. We provide a training and accreditation environment for GP Registrars, and multidisciplinary training for nursing students, paramedics and clinical pharmacists. As well as two GP Trainers and a GP Training Programme Director, a nurse mentor and paramedic FCP supervisor, our Team includes the Weald PCN in Community Education Facilitator and multidisciplinary education leads, and an educational project lead for Kent and Medway training hub.     Post Title: Paramedic Practitioner

    Hours: Hours are negotiable and flexible, full or part-time considered.

    Salary: Dependant on experience but based on Band 7-8 agenda for change rates

    Job Summary

    The post holder will primarily offer paramedic practitioner skills and work collaboratively with GPs and the clinical team within the boundaries of Practice protocols and guidelines.

    The post holder, acting within their own professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the Practice.

    Key responsibilities and duties

    This is not an exhaustive list and the role will evolve and expand as the successful candidate develops experience and confidence in working in General Practice.

    Clinical practice

    The post holder will be required:

    To assist GPs by seeing and triaging patients requesting same day appointments and to then assess, diagnose, and develop management plans and care for those patients presenting with an undifferentiated diagnosis in a booked clinic which may involve both face to face and telephone consultations. This will include management of patients with both acute and chronic conditions. The following exclusions will apply initially: children aged < 1 year, pregnant women, skin lesions and gynaecological/womens health problems

    To liaise with the duty doctor to ensure prescribing of appropriate medication.

    To deal appropriately with patients presenting at the Surgery who require immediate or emergency assessment and to initiate effective emergency care

    To initiate, request and interpret appropriate investigations and referrals

    To undertake some home visits within the practitioners competence.

    To assess patients needs from a physiological and psychosocial perspective and to plan clinical care accordingly.

    To assess and develop management plans for patients with complex needs ( including the housebound and those on the Frailty register)

    To support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

    To develop integrated working with the GPs and Multi-disciplinary team within the Surgery and community and to coordinate meetings as required.

    To provide hands on treatment room assistance if needed

    Venepuncture of patients being reviewed

    Administration and Professional Responsibilities

    Ensure accurate notes of all consultations are recorded on the clinical computer system

    Ensure accurate completion of all necessary documentation associated with patient health care

    Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audits

    Restock and maintain tidiness of clinical areas and consulting rooms when required

    Monitoring laboratory results and actioning according to a defined protocol ( when time permits)

    Reviewing and triage of Docman documents ( when time permits)

    Training and Personal Development

    There will be supervision and mentoring provided by one of the GPs or the Advanced Paramedic Practitioner for each clinical session undertaken. Learning outcomes will be discussed and recorded by the practitioner.

    Personal development will be encouraged and supported. Training, development needs and performance will be assessed by an annual appraisal.

    It is the individuals responsibility to meet requirements for continued professional registration, remain up to date with new and guidance, developments and to complete mandatory training.

    Attendance at in-house clinical governance and other training meetings will be expected

    Liaison

    Work closely with the all clinical and non-clinical members of the Practice Team to promote the smooth running of the Practice

    Establish and maintain good working relationships with the wider primary health care team, community health workers and secondary care departments

    Confidentiality

    All confidential information relating to patients, their carers, practice staff and other health care workers must be regarded as strictly confidential. Information relating to the Practice as a business organisation must also be treated as confidential

    Adherence to the Practice confidentiality and information governance policies is expected at all times.

    Health and Safety

    The post holder has a responsibility to comply with the Health and Safety at work Act 1974 and should identify risks involved in work activities and undertake activities in a way that manages these risks. There is an expectation that all duties will be carried out in a manner that promotes the principles and practices of infection prevention and control in compliance with national standards and Practice policies.

    Experience

    Essential

    • Post registration qualification to evidence autonomous practice
    • Understanding of patient management in primary care setting

    Desirable

    • Experience of working in a multidisciplinary team in an acute Primary Care setting
    • Sound knowledge of local primary care services
    • Administration of medications within Patient Group Directives

    Qualifications
