Job summary

An exciting opportunity has arisen to recruit a Band 7 Mental Health Pathway Lead within the Older Adult Outreach Team, working across the South Staffordshire West/East locality.

The Outreach Team provides a 7-day service (08:00-18:00), delivering a proactive and responsive approach to supporting older adults who require intensive intervention and home treatment as an alternative to hospital admission.

We are looking for a motivated and experienced practitioner with specialist knowledge in older adult mental health. The post holder will work with individuals experiencing a wide range of functional mental health conditions and dementia, including those with complex and challenging presentations.

This role combines clinical expertise with operational leadership responsibilities, working closely within a multidisciplinary team and alongside partners across physical health, social care, and primary care services

Main duties of the job

  • Deliver high-quality, person-centred care to individuals referred to the Older Adult Outreach Team
  • Provide day-to-day clinical and operational leadership, including supervision of staff
  • Support effective service delivery and contribute to team performance
  • Participate in Collective Leadership and Quality Improvement initiatives
  • Work collaboratively with MDT colleagues and external partners, including GPs and social care
  • Maintain accurate clinical records in line with organisational standards
  • Ensure compliance with mandatory training and take responsibility for ongoing professional development

  • First level professional Nursing /AHP/Social Work qualification.
  • Post-registration experience and/or evidence of formal courses and training at post graduate diploma level

Experience

Essential

  • Experience of managing multi- disciplinary teams
  • Experience of effectively initiating, developing, implementing and evaluating operational processes.
  • Demonstrable experience in performance management
  • Experience in the direct line management of health or social care staff
  • Significant demonstrable post qualification experience at Band 6 working in a mental health / specialist inpatient setting.
  • Demonstrable experience of effective multi-disciplinary team working.
