Pathways Care Coordinator (East Warrington PCN)

NHS

Job Description

Job summary

Are you passionate about making a real difference in people's lives? We are looking for a compassionate and proactive Care Coordinator to join East Warrington Primary Care Network (PCN), working collaboratively across our three GP practices to support patients in accessing the care and services they need.

In this rewarding patient-facing role, you will play a key part in improving health outcomes and reducing health inequalities within our local community. Working with both adults and children, you will help individuals navigate a range of healthcare pathways, initially focusing on neurodevelopmental and cancer care pathways, while also supporting other priority areas in response to patient and practice needs.

This is an opportunity to have a meaningful impact on the lives of some of the most vulnerable members of our community, including individuals waiting for neurodevelopmental assessments, people living with long-term conditions, and those navigating cancer care pathways. No two days are the same, offering a varied and fulfilling workload where your support can genuinely change lives.

Interviews will be held on Friday 17th July 2026.
