Job summary

This is an exciting opportunity for an experienced and motivated onsite clinical pharmacist to join our growing, supportive team.

Please note this is an onsite role and not remote working.

You should be able to work independently and demonstrate a high standard of safe clinical decision making with a broad and relevant knowledge base.

The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the Quality and Outcomes Framework and Investment and Impact Fund where appropriate.

We are looking for a candidate who is a qualified Independent Prescriber.

Main duties of the job

Provide support to general practice staff with regards to prescription and medication queries and help support the repeat prescription system

Lead on the monitoring of practice prescribing against national guidelines and formulary recommendations,including High Risk Medication prescribing/monitoring

Undertake clinical medication reviews with patients and make recommendations on prescribing and monitoring

Work with care home staff to improve safety of medicines ordering and administration

Attend and refer patients to multidisciplinary team meetings

Where appropriate and required, provide patient facing clinics and/or phone consultations for those with questions and concerns about their medicines in the practice

Manage caseload of patients with common/self-limiting/minor ailments while working within a scope of practice and limits of competence

Set up and manage systems to ensure continuity of medicines supply to high risk groups of patients

Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components

Analyse, present and interpret medicines data to highlight issue and risks to support decision making

Assist in the transition to eRD

Ensure the practice is compliant with CQC standards where medicines are involved.

Job responsibilities

Providing medicines optimisation to patient population

Work with the general practice team to ensure the practice is compliant with Care Quality Commission (CQC) Standards where medicines are involved.

Undertake audits and support the team with safe prescribing.

Monitoring patients with complex long term conditions such as hypertension or diabetes

Undertaking Structured Medication Reviews (SMRs) as stipulated in the Network Contract DES. Review medication for newly registered practice patients.

Management of self-limiting ailments; signposting to community pharmacy and referring to GPs or other health care professionals where appropriate.

Medicine information to practice staff and patients

Unplanned admissions; work with multidisciplinary review teams to manage medicines-related risk for readmission and patient harm. Put in place changes to reduce the prescribing of high risk medicines to high-risk patient groups.

Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging those up needing a review.

Support and work with the CCG Medicines Optimisation in Care Homes team to undertake SMRs with patients with multi-morbidity and polypharmacy living in long term residential care.

Take a leading role in the contractual requirements to support frail patients with their medication including patient identification and undertaking SMRs.

Implementation of local and national guidelines and formulary recommendations

Provide education and training to primary healthcare team on therapeutics and medicines optimisation. Contribute to clinical meetings and case reviews. Provide training to students where appropriate.

Support Public health campaigns and provide specialist knowledge.

Person Specification

Qualifications

Essential

  • Mandatory registration with General Pharmaceutical Council
  • Masters degree in pharmacy (MPharm) or equivalent
  • Independent Prescriber

Desirable

  • Clinical diploma

Experience

  • Minimum of 3 years in post

Knowledge and Skills

  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
  • Understanding of the nature of general practice
  • Excellent interpersonal, influencing and negotiating skills
  • Excellent written and verbal communication skills
  • Able to work under pressure and meet deadlines
  • Work effectively independently and as a team member
