Main duties of the job

At its core, you will work as a member of the in-house clinical teams and provide a critical role of delivering appointments and care to the registered list of patients. You will play a key role within the practice, helping to ensure patients get the right care they need from the right person at the right time. You will manage admin such as blood results and prescriptions and be part of a team to collectively manage shared admin alongside other GPs and other staff members. Responsibilities will reflect requirements of the NHS GP Contracts, and as such may be subject to change as services adapt. This post offers the opportunity for role and career development, especially with regards to training and supervision of the wider MDT as the team and service evolves. The role may also involve complex care depending on your interests and skills.

About us

Islington GP Federation (IGPF) is a growing organisation representing 31 practices; we have established ourselves as a leader in new ways of working, including running Islingtons extended access primary care services (I: HUB) as well as supporting the Islington Primary Care Networks (PCNs). Our current range of services include the Extended Access Service, I: HUB, Community Ear, Nose and Throat (ENT), Integrated Community Gynaecology, practice-based pharmacists and a range of practice support mechanisms.

IGPF works very closely with a range of partners including the regional commissioning group, NHS England, Healthy London Partnership, Public Health, local hospitals such as UCLH, Whittington Health and the London Borough of Islington.

IGPF is the host organisation for the Primary Care Network (PCN) workforce and the Islington Training Hub and has been working for over three years to create training and development programmes that meet the needs of staff working in primary and community care settings.

IGPF runs four Islington GP Practices-Northern Medical Practice, Barnsbury Medical Practice, City Road Medical Centre and Hanley Primary Care Centre

Job responsibilities

Triage sessions and clinics

We operate a total triage system and you will have dedicated triage sessions where you are also duty doctor. Once a patient has been triaged and assessed as appropriate, they will be scheduled for an initial consultation, either in person at the practice or via telephone/virtual consultation.

Clinicians document their consultations within EMIS, the primary care electronic record system. This ensures seamless access to the patients medical history and timely communication with other healthcare providers, such as GPs or specialists, ensuring follow-up actions are visible to all involved in the patient's care. Templates are available within EMIS to streamline documentation, and each clinician will receive an induction to familiarise themselves with the system to optimise its use.

PCN GPs are expected to review incoming clinical results within EMIS and take appropriate action for any abnormal findings. They are also responsible for addressing any clinical queries arising from virtual or face-to-face consultations.

In cases where a procedure is deemed necessary, the appropriate local clinical pathways will be followed. This includes ensuring informed consent and conducting any necessary pre-operative investigations.

Clinical sessions are structured for 4 hours and 10 minutes. Initial consultations and follow-up consultations can be varied between 12 and 15 minute slots. Additional time may be allocated for specific needs, such as using interpreters or supporting patients with additional requirements.

At the end of each clinical session, admin time is dedicated for addressing results, preparing referral letters, completing tasks, and ensuring that all documentation and follow-up actions are completed.

Other duties include:

  • To provide the full range of NHS Primary Medical Services to the patients including clinical examination, diagnosis, treatment and referral as appropriate.
  • To be personally accountable for professional and ethical actions and practices within a legal and ethical framework that adheres to the GMCs requirements and local policies and procedures.
  • To ensure that prior to any course of action involving individuals/groups, their informed consent or other valid authority has been obtained.
  • To ensure that record-keeping is managed through the creation and maintenance of concise, contemporaneous computerised records, and that all care is recorded accurately and in a timely fashion. Notes should include a Problem READ coded, history, examination where appropriate and a clear management plan.
  • To interpret and explain complex facts in relation to a patients condition, taking appropriate action and giving advice in a professional manner to team members, and when appropriate to family and other carers, in such a way that protects and preserves the patients dignity.
  • To have continuing responsibility for Adult and Child Safeguarding in all cases of suspected or confirmed abuse, and to ensure compliance with local and national guidelines, policies and procedures.
  • To ensure that concerns and identified potential risks are referred to the appropriate Agency immediately.
  • To assess and diagnose across a wide range of conditions, order investigations, plan and provide treatment as needed and discharge patients.
  • If an examination is required, a chaperone will be provided should the patient request one. It is expected that all patients undergoing an examination will be asked if they would like a chaperone.
  • Be aware of where the emergency equipment and medication are kept at the start of each clinic
  • To record in the medical record why investigations are being undertaken. To analyse the results and record in the medical record what actions are required.
  • If a referral to secondary care is required, the clinician will complete the relevant referral letter and ensure the onward referral is completed or tasked to the admin team depending on agreed guidelines
  • If the patient needs a 2 week wait (2WW) referral, the clinician will complete the referral form in clinic and do the eRS referral themselves to ensure it is completed within 24 hours.
  • To assess, tolerate and manage risk and deal with uncertainty
  • To use evidence-based practice to develop and maintain high quality, clinically effective and cost-effective primary care.
  • To prescribe in accordance with regulations and with the post holders training and registration and in accordance with CCG, NHS England, NW London Prescribing Team, Practice Based Commissioning and local formulary.
  • To maintain full registration with the General Medical Council at the employees own expense.
  • To maintain professional indemnity which covers the scope of work

Attendance at clinical meetings

All clinicians meet weekly and there is an expectation that all within the service are expected to attend and contribute to governance procedures.

Qualifications

Essential

  • Full Registration with the General Medical Council
  • GMC Specialist Register for General Practice
  • Membership of a medical defence organisation

Desirable

  • Relevant Higher Degree e.g. MD, PhD, MSc or other additional clinical qualifications

Experience

Essential

  • Experience of working independently
  • Experience of working in UK General Practice.
  • Experience of working with diverse and vulnerable groups, including use of interpreters.

Desirable

  • Experience of clinical triage
  • Delivery of clinical audit
  • QOF management
  • Experience of working in Islington/ NCL general practice
  • Experience of working in at-scale systems
  • Experience of safeguarding leadership/ desire to take this on
  • Supervision of other roles and staff members
