Job summary

Are you looking to join a dedicated and caring team who support local patients when they need it the most?

We have a great opportunity for a part time Physiotherapist to join our Patient Flow team to provide compassionate, person-centred, holistic rehabilitation services to palliative care patients. This is a key role that supports our patients, helping to improve their mobility, independence and quality of life both in an inpatient and community setting. You don't need to have palliative care experience we welcome applications from a range of clinical areas.

This is a part-time position working 22.5 hours per week and we are open to discussing the working days at interview however it is expected that it will be between Monday to Friday.

Main duties of the job

This role will cover:

  • Undertaking holistic, special assessments of patients and develop dynamic treatment plans which focus on maximising function.
  • Providing education and support to carers when planning for a patient to be transferred from the IPU to another location and be actively involved in developing treatments plans around self-management.
  • Regularly attending MDT meetings, family meetings and case meetings to provide specialist advice.
  • Keeping up to date patients notes and records in accordance with Hospice policy.

In order to fulfil this role, you will need to have:

  • A recognised qualification in physiotherapy and be registered with the HCPC
  • Experience of delivering group based and individual physiotherapy
  • The ability to assess the needs of individuals and groups including the whole family in order to work with them as a unit
  • Excellent communication skills, both verbal and written
  • A flexible, supportive and calm approach to work

Job responsibilities

The Physiotherapist is an integral member of the Therapy Services Team at St Leonards Hospice and a key part of the wider Patient Flow Team. The role provides person-centred, holistic rehabilitation for people receiving palliative care, including patients with cancer, long-term neurological conditions, and other progressive life-limiting illnesses.

Main duties and responsibilities

Clinical

1. Provide an expert and skilled therapy service to patients under the care of the Hospice.

2. Undertake holistic, specialist assessments of patients and develop dynamic treatment plans. Implement and deliver excellent treatment to patients focussing on maximising function.

3. Along with other allied health professions. Identify and work with patients at as early a point as possible to maintain or develop independence in activities of daily living and routines.

4. Work as part of the MDT in meeting patients personal goals around their needs and priorities.

5. Provide education and support to carers when developing discharge plans. Actively involve both patient and carer in developing treatment plans around self-management.

6. Be responsible for the safe moving and handling of patients during mobility and transfers, including the use of equipment such as hoists.

Managerial

1. Be involved in the supervision of physiotherapy students whilst on placement at the Hospice.

2. Keep up to date patient notes and records in accordance with Hospice policy and with CSP standards.

3. Place an active role in developing/ contributing, reviewing and updating policies concerning physiotherapy/ rehabilitation as requested.

Communication

1. Use formal and informal communication with staff and external agencies to maintain effective information-sharing and enhance patient care.

2. Communicate difficult information to patients in emotionally charged situations with tact and empathy, encouraging participation in therapy through negotiation, reassurance and support.

Education

1. Organise and deliver training courses to healthcare professionals including those internal and external to the organisation, in collaboration or consultation with the Occupational Therapist and Clinical Practice Facilitator.

2. Provide teaching both within the departments and to the wider multidisciplinary team on issues around physiotherapy service and manual handling.

3. Identify, initiate and undertake audit and research projects pertinent to occupational therapy, physiotherapy, rehabilitation and the AHP team. Comply with all Care Quality Commission compliance requirements.

Qualifications

Essential

  • Recognised qualification in physiotherapy (BSc/ MSc)
  • Registered with the HCPC
  • MCSP or equivalent
