We are delighted to offer an exciting opportunity for a compassionate and motivated Practice Nurse to join our friendly and forward-thinking GP practice for three days per week (0.6 FTE). This is a rewarding role within a supportive primary care team committed to delivering high-quality, patient-centred care to our local community.

As a valued member of our multidisciplinary team, you will play a key role in providing a broad range of nursing services, including chronic disease management, immunisations, cervical screening, health promotion and preventative care. You will have the opportunity to build meaningful relationships with patients while working autonomously and collaboratively with experienced clinicians and administrative colleagues.

The Practice Nurse will deliver high-quality nursing care within a busy primary care setting, supporting patients across all age groups. The role includes running nurse-led clinics, supporting the management of long-term conditions such as asthma, diabetes and hypertension, delivering vaccinations and immunisations, undertaking cervical screening, and providing preventative health advice. You will work both independently and collaboratively as part of a multidisciplinary team to ensure safe, effective, and patient-centred care, while contributing to the smooth day-to-day running of clinical services.

Job responsibilities

Main Duties and Responsibilities

  • Deliver evidence-based nursing care to patients within the practice.
  • Undertake chronic disease reviews including asthma, COPD, diabetes and hypertension.
  • Administer vaccinations and immunisations in accordance with national guidelines.
  • Perform cervical cytology screening.
  • Provide wound care and dressing management.
  • Conduct health promotion and lifestyle advice clinics.
  • Maintain accurate patient records using the clinical system.
  • Participate in infection prevention and control activities.
  • Work collaboratively with the wider multidisciplinary team to improve patient outcomes.

Person Specification

Qualifications

Essential

  • Registered Nurse with current NMC registration
  • Must have experience working within General Practice
  • Competent in immunisations and vaccinations
  • Cervical Cytology qualification
  • Experience of chronic disease management
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Desirable

  • Experience using SystmOne
  • Experience in respiratory or diabetes management
