Within 20minutes of Ipswich, Haven Health and Howard House surgery's in Felixstowe have multi-disciplinary teams who are serious about healthcare and achieving the best for all their patients but who like to work in a supportive and fun atmosphere. We are seeking to recruit an enthusiastic and flexible Practice Paramedic to work in the community treating both Haven Health and Howard House patients.

Suitable candidates must be able to work flexibly and autonomously in the community supporting both our housebound and care home patients. It is not necessary to hold a prescribing qualification for this role but it is however, necessary to be a proactive team member

The vacancy is for approx. 30 hrs a week, over 4 days; however, flexibility is crucial to accommodate training, holidays and occasional sickness.

We have established teams with a strong focus on clinical excellence, customer service and communication. We would love to hear from candidates with Primary Care experience.

Main duties of the job

The main duties will be treating both housebound and care home patients across 2 surgeries. Leading on the various vaccination campaigns. Completing referrals where applicable. Monitoring long term conditions and working closely with GP's/debriefing following home visits.

The home visiting team consists of paramedics and a community HCA.

Job responsibilities

To provide high-quality and clinical excellence to the patients of Haven Health and Howard House GP surgeries; diagnosing and prescribing treatments and medication while working within defined procedures and protocols as laid down by the GPs.

To work as part of a clinical team, with paramedic's, GPs, Advanced Nurse Practitioners, Practice Nurses and Healthcare Assistants across both practices. Working alongside non-clinical team members

To be responsible for seeing patients across two practice sites, care homes and for visiting patients in their place of residence, who present with a wide range of minor illness, urgent and same day requests for treatment and chronic disease management; providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

The post holder will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. There will be the need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.

Key Responsibilities

Ensure all medical notes are written contemporaneously and succinctly

Provide assessment, diagnosis and treatment at first point of contact by attending to patients according to their needs in the patients homes/Care homes. Act as an autonomous clinician, with the support and guidance from the GPs

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.

Accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.

Recognise, assess and refer patients presenting with mental health needs

Telephone triage as appropriate for clinics and home visits and care home ward rounds and provide access to GPs as necessary.

Evaluate clinical information from examination and history taking and initiate appropriate treatment and / or referral, including admission to hospital if required.

Signpost appropriately to other services in the identified healthcare pathway and to refer patients to appropriate care settings, within locally agreed healthcare pathways.

Maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.

Advise and support members of the public on healthy lifestyles, health promotion and disease prevention.

Provide clinical advice to other members of the team where appropriate.

Possibility to work in the clinic setting as required

Any other duties within the scope of the post holder as requested by the management team.

  • Ideally with previous experience of working in Primary Care. Although this does not preclude those having worked in secondary care.
