Job summary

We are looking for a motivated, patientcentred Pharmacist to join our friendly, forwardthinking coastal practice.

In post, you'll play a key role in medicines optimisation, longterm condition management, and supporting safe, effective prescribing.

This role is ideal for someone who thrives in a collaborative environment, enjoys problemsolving, and wants to make a meaningful impact on patient care within a supportive environment.

The Role includes:

Medication reviews structured medication reviews (SMRs), polypharmacy reviews, and medicines reconciliation

Longterm condition support contributing to management of hypertension, asthma, COPD, diabetes and more

Clinical support assisting GPs with complex prescribing queries and safety monitoring

Quality improvement contributing to audits, QOF, IIF and medicines safety initiatives

Patient education supporting patients to understand and manage their medicines

Collaborative working working closely with GPs, nurses, HCAs, other multi-agency colleagues

You will need:

To be a GPhCregistered Pharmacist

Have experience in primary care, community, hospital or PCN settings

Have strong clinical reasoning and communication skills

To be a proactive, organised, supportive team player

To hold an Independent Prescriber qualification (or be willing to work towards it)

Main duties of the job

As a Practice Pharmacist, you will deliver structured medication reviews, support longterm condition management, provide clinical advice to patients, and work closely with our GPs and nursing team to ensure safe, effective prescribing.

You will also play a key role in supporting our experienced dispensary team with medicines queries, clinical oversight and workflow safety.

Job responsibilities

Key Responsibilities:

Medicines Optimisation.

Lead medicines optimisation work across the practice

Review prescribing trends and support safe, cost effective prescribing

Provide advice to clinicians on formulary, guidelines, and best practice

Medication Reviews.

Conduct structured medication reviews SMRs

Support polypharmacy management and de prescribing where appropriate

Identify interactions, contra indications, and adherence issues

Repeat Prescribing Management.

Oversee repeat prescribing processes and safety checks

Resolve medication queries from patients, clinicians, and dispensary staff

Support synchronisation, medication switches, and prescription rationalisation

Clinical Support.

Provide clinical advice during patient consultations within competence

Support chronic disease management

Assist with monitoring, titration, and medicines related care plans

Care Home and High Risk Medicines Work.

Conduct medication reviews for care home residents

Support monitoring of high risk medicines

Ensure safe systems for monitoring, recalls, and follow up

Clinical Governance & Safety.

Lead medicines safety audits, incident reviews, and quality improvement work

Support compliance with CQC, NICE, and local prescribing guidelines

Maintain uptodate SOPs and contribute to riskmanagement processes

Patient Education & Support.

Provide advice on medication use, side effects, and adherence

Support self management and lifestyle interventions

Handle escalated patient queries sensitively and professionally

Collaboration with MDT.

Work closely with GPs, NPs, nurses, HCAs, and dispensary teams

Participate in MDT meetings and contribute to clinical decision making

Support safe transitions of care and medicines reconciliation

Prescribing.

Prescribe within scope of competence as an Independent Prescriber

Manage long term condition clinics or medicines focused pathways

Ensure safe documentation, coding, and follow up

Data Quality and Reporting.

Ensure accurate coding of medication reviews and interventions

Support QOF, IIF, and prescribing related reporting

Contribute to audits, dashboards, and performance monitoring

Person Specification

Qualifications

Essential

  • To be a GPhC registered Pharmacist.
  • To have experience in primary care, community, hospital or PCN settings.

Desirable

  • To be a GPhC registered Pharmacist.
  • To have experience in primary care, community, hospital or PCN settings.
