Main duties of the job
The aims of this post are to:
Increase the number of physical health checks performed on people with SMI and substance misuse who may be at increased risk of COVID-19 and its subsequent complications.
Increase the overall number of physical health checks performed on people with SMI in Haringey from 21% in 2018 to the national target of 75% as of 2026.
Encourage a more holistic approach to healthcare for patients through collaborative working with North London Foundation Trust and the Haringey Wellbeing Network at Mind in Haringey.
The post holder will work closely with the SMI Community Mental Health Allied Professional, HAGA services and service lead to deliver on key national and local strategies to reduce unwarranted variation in the physical health outcomes of patients with SMI and substance misuse by:
Increasing the proactive identification, monitoring and management of physical illness in this patient cohort.
Rapidly identifying at-risk patients on the SMI register, impacted by or with potential higher risks to healthcare complications as a result of COVID-19.
Improving engagement of SMI patients with primary and community care, including referral or signposting to appropriate services.
Supporting practices maintain accurate SMI registers.
Identifying and coordinating patient care with the Haringey Wellbeing Network.

Job responsibilities
As a member of Haringey GP Federation Primary Care Clinical Team, the post holder will provide high quality clinical care and operational support to the Haringey Serious Mental Illness (SMI) Physical Health Check Service and GP @ The Grove and HAGA Service (substance misuse). You will be working with several providers across several sectors: primary care, community health, hospital, social care and the voluntary sector.
Your primary focus will be conducting SMI Physical Health Checks across Haringey GP surgeries, where your work will directly help optimise the health of vulnerable patients, reducing their risk of developing serious, life-threatening conditions such as heart attacks and strokes.
Additionally, you will play a vital role in supporting a specialised GP clinic hosted within HAGA (Haringey Alcohol Service) once every two weeks, providing essential healthcare continuity to patients overcoming alcohol addiction.

Person Specification
Qualifications
Essential
- Good standard of general education including Maths and English at Grade C and above (or equivalent).
- Health and Social Care qualification such as Care Certificate or NVQ level 2 or 3.
- Good understanding of spoken and written English.
- Desire to develop skills and qualifications under continuous professional development.
- Knowledgeable use of the clinical system EMIS.
