Job summary
The Referral Coordinator plays a vital role in driving occupancy across Isabella's Homes by managing referrals from local authorities, NHS partners and commissioners.
You'll coordinate assessments, liaise with professionals and ensure referrals progress efficiently from enquiry through to admission.
Main duties of the job
Key ResponsibilitiesManage incoming referrals.
Build relationships with commissioners and social workers.
Coordinate assessments and viewings.
Track referral pipeline and occupancy.
Prepare placement proposals and costings.
Work alongside Operations and Registered Managers.
Ensure referral documentation is accurate.
Monitor KPIs and reporting.
Attend networking events where required.
Support mobilisation of new services.

Salary: £32,000£38,000 per annum + Performance Bonus
