Job summary
We are seeking a motivated Community Registered Nurse to join our integrated Community Nursing Service in North East Essex.
This role offers the opportunity to deliver high-quality, patient-centred nursing care within patients' own homes, supporting independence and preventing avoidable hospital admissions. You will work as part of a multidisciplinary neighbourhood team, managing a varied and rewarding caseload across the community setting
Our service operates 08:00-18:00, 7 days a week.
You will be responsible for assessing, planning, implementing and evaluating evidence-based nursing care, including:
Complex wound care and pressure ulcer management
Leg ulcer care and Doppler assessment
Catheterisation (male/female/suprapubic)
IV, subcutaneous and intramuscular medication administration
PICC/Hickman line management
Palliative and end-of-life care including syringe drivers and symptom control
Continence and bowel management
Supporting referrals and coordinating multi-agency care
You will work autonomously while being supported by senior colleagues, and will contribute to clinical governance, audit and service improvement

Main duties of the job
To take responsibility for assessing, planning, implementing and evaluating evidence based care for patients.
To provide ongoing nursing care and support to patients and carers, promoting the role of the nurse and clinical excellence.
To identify patients whose conditions become unstable and liaise with doctors and acute services appropriately.
To network with other professionals in order to co-ordinate a multi-disciplinary approach to care provision.
To be responsible for multi-agency care documentation in line with current practice.
To ensure that patients are able to make an informed choice about their current and future care needs.
Ensure that care is patient centred and confidentiality is maintained in line with Trust policy.
To prioritise own workload and work autonomously when necessary.
