Job summary
We are looking for an enthusiastic Rehabilitation Assistant to join the Sutton Health & Care Maximising Independence Service. The Maximising Independence Service comprises of community Frailty, Falls Prevention and uni disciplinary physiotherapy pathways. The services include community centre based Frailty MDT Clinic, Falls Prevention Groups and home visits.

Main duties of the job
The main duties of the job include supporting the clinicians, especially the therapists to deliver rehabilitative intervention to our patients in the clinic/gym or in their usual place of residence. This would involve delivering individually tailored exercise/rehabilitation programmes, joint sessions with clinician, leading/supporting the Falls prevention/ Ageing-well exercise classes as well as equipment delivery, fitting or review.
You will be part of the MDT team that comprises of a Nurse, Physiotherapists, Occupational therapist, Assistant Practitioners, Rehabilitation assistants and Administrative staff. You would also be expected to support with administrative duties within the team where needed.
You, as part of the team, will work closely with the other community services within Sutton Health and Care that help to prevent admission from the community, accelerate discharge from emergency department and acute medical wards and provide supported discharge through bed and home-based intermediate care. You will be supported by the Clinical Manager and the clinical staff.

Job responsibilities
You will undertake delegated duties to facilitate patient participation in their treatment/rehabilitation programme and/or provide support as directed by a therapist in a range of settings including domiciliary and group exercise classes in the local environment, by performing tasks which do not require the skills of a qualified therapist or nurse.
Responsibilities will also include delivery and fitting of equipment, administrative support as well as other task(s) deemed appropriate to support service delivery and patient care.

Qualification and Training
Essential
- oNVQ Level 2/3 in health and/or social care
- oWritten, verbal and numeric skills to GCSE or similar level
Desirable
- oHigher level degree qualification in relevant subject
- Care certificate
Experience
Essential
- oExperience working in a caring role
- oExperience working in the NHS
- oExperience in patient documentation
Desirable
- oWorked as a therapy or healthcare assistant
- oExperience of working as a member of a team within health or social care
- oExperience working in the community setting
- oExperience running/ supporting an exercise class
Knowledge, skills and abilities
Essential
- oGood written, spoken and non-verbal communication skills
- oBasic IT Skills. e.g. MS Word, Outlook, Emails
- oPhysically fit to meet full demands of the post subject to occupational health clearance
Desirable
- oKnowledge of different types of mobility aid
- oKnowledge of different OT aids

- Car Driver with valid UK licence
