We are looking for a Band 3 Rehabilitation Support Worker to join our Community Enhanced Home-Based Rehabilitation Service.
The Merton Enhanced Home-Based Rehabilitation Team provides a seven-day service (8am to 6pm) that supports people to return home safely after a hospital stay when they no longer require care in an acute hospital bed.
Working collaboratively across health and social care services, the team helps people settle back into their home environment through timely assessments and rehabilitation support. Our aim is to promote safety, wellbeing, and independence at home, while helping to reduce avoidable hospital readmissions.
Based at Merton Civic Centre, the team delivers rehabilitation support in patients' homes and other community settings across the Merton locality.As part of a multidisciplinary team, you will work closely with Physiotherapists, Occupational Therapists, and other colleagues to support people in achieving their rehabilitation goals and improving their wellbeing following illness, injury, falls, or a hospital admission.

Main duties of the job
The Rehabilitation Support Worker (RSW) is an integral member of the multidisciplinary team, supporting the delivery of rehabilitation and patient-centred care under the guidance and supervision of Allied Health Professionals.
You will be supported to develop the knowledge, skills and competencies required for the role through training and competency-based assessments relevant to your area of practice.
You will promote independence and support patients to achieve their rehabilitation goals. You will also contribute to service quality and support the team in meeting key performance indicators.
Key responsibilities include:o Working alongside multidisciplinary team members to support the care, rehabilitation and wellbeing of patients.o Contributing to ongoing patient assessment by accurately recording, monitoring and reporting clinical and behavioural observations to registered practitioners.o Carrying out delegated rehabilitation, nursing and therapy interventions following appropriate training and competency assessment.o Reviewing and working in accordance with individual care and rehabilitation plans before each intervention.o Delivering delegated rehabilitation programmes independently within agreed protocols and reporting progress, concerns and outcomes back to the supervising therapist or nurse.o Maintain accurate records and contributing to effective communication within the multidisciplinary team.o Support the delivery of a safe, effective and high-quality service.

Experience
Essential
- Experience of working in a health or social care setting.
- Experience of working as part of a team
- Experience with the use of Microsoft office and a variety of databases
Desirable
- Demonstrable understanding of the roles of members of the MDT
- Experience as a therapy assistant
- Experience with the NHS patient information database (RIO)
Skills
Essential
- Good written and spoken communication skills
- Demonstrate understanding of the role of a rehabilitation assistant.
- Demonstrate an acceptable level of numeracy and literacy
- An awareness of risk management and issues relating to working in people's homes.
- Knowledge of multi- agency services available and processes for referral
- Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being.
- Knowledge of the NHS
Desirable
- Knowledge of local health needs strategies
Qualifications
Essential
- Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or equivalent
- Willingness to undertake further training as appropriate
Desirable
- GCSE grade 9 - 4 or equivalent in English and Maths.
- First aid qualification
