SECONDMENT or FIXED TERM (6 months )
An exciting opportunity has arisen for a Senior Case Manager at the Humber and North Yorkshire Health and Care Partnership Mental Health, Learning Disabilities and Autism Collaborative.
Please ensure approval for secondment is sought prior to application being submitted.This is a part-time post of 22.5 hours (3 days per week).
Main duties of the job
The post holder will work as part of a dynamic team in delivering an effective service; supporting managers and staff across Humber and North Yorkshire to ensure safe and effective care pathways are in place for service users of out of area hospital placements within the NHS and Independent sectors, including rehabilitation, adult and older adult acute and PICU placements.
This will include:
Undertaking face to face quality visits which will include travel outside of the Humber and North Yorkshire region.
Agreeing robust discharge plans in line with national guidance.
Liaising with key partners from the system to ensure local services can meet individual need and oversee the repatriation of individuals.

Skills and Competencies
Essential
- Demonstrate an understanding of the requirements of quality in healthcare and commissioning healthcare services.
- Ability to negotiate on difficult and controversial issues including performance and change.
Desirable
- Excellent programme management skills
Qualifications, Education & Training
Essential
- Registered Mental Health / Learning Disability Nurse or other registered health or social care professional
- Educated to master's level or equivalent level of experience of working at a senior level in specialist area.
Desirable
- Prince 2 and Managing Successful Projects.
Knowledge & Experience
Essential
- Experience of managing clinical risks and reporting.
- Experience of drafting briefing papers and correspondence for a senior audience
Desirable
- Experience of working in quality assurance/quality improvement
