Job summary

The purpose of this role is to take a lead within the wider multidisciplinary team to effectively assess and monitor the physical health of patients receiving treatment that requires monitoring of their physical health and who live in the community. The team will facilitate and/or deliver evidence-based interventions as needed to deliver safe care and improve health as needed. The post holder will:

Work closely with the Clinical Team Manager (CTM) in overseeing efficiency and effectiveness of care provided to a case load of patients.

Present as a positive and professional role model to all staff within the unit, striving to maintain a philosophy of service user involvement in continuously improving the service.

Work as an integrated member of the clinical team, participating in all aspects of day to day service provision.

Work with a defined caseload of service users and as part of a multi-disciplinary team, providing assessment, care planning and care delivery. This will include a high level of skill and competence in undertaking physical and mental health assessments in the context of the mental health care pathway.

Work collaboratively with other professionals involved with those accessing the service to support individual goals and effective service delivery; this will include GP practices, medical colleagues and wider health and social care partners.

Main duties of the job

The service has an opportunity to implement service improvements and this temporary role is part of implementing these changes. The role will predominantly work alongside the team to ensure all service users on the caseload have received an annual physical health check and any issues have been escalated and addressed. The role will work alongside and in conjunction with the main care teams for the service user to ensure they also receive an annual review of the prescription and treatment with the prescribing medic.

There will be a need for the post holder to be skilled in assessing mental and physical health and completing associated care plans and risk assessments for the service users.

The post holder will work within the Clozapine/amber drug and Annual Health Check clinics facilitated by the service across 4 LYPFT sites.

The post holder will provide day to day leadership to the whole team and take on the "in charge" role to ensure the clinics across all the sites that day run smoothly.

Qualifications

Essential

  • Registered Nurse - RMN or RN

Skills

Essential

  • Excellent communication skills.
  • Excellent skills in assessment, planning and delivery of evidence-based care / interventions based upon the individual needs of the service user relevant to their mental and physical health needs.
  • Full driving licence and access to a car for work purposes.

Experience

Essential

  • Experience and knowledge of working with patients who are prescribed Clozapine and Amber drugs and Annual Physical Health Checks
  • Experience and knowledge of the problems and difficulties experienced by those with mental health illness especially SMI

Knowledge

Essential

  • Demonstrates a high level of understanding of the national physical and mental health integration agenda and ability to contribute to related service development initiatives.
