Are you an experienced and compassionate nurse looking to make a real difference to the lives of some of the most vulnerable patients within the community?

We are seeking an enthusiastic and highly skilled senior nurse to join our Integrated Neighbourhood Team, working across a Primary Care Network of three GP practices. This is an exciting opportunity to work collaboratively within a multidisciplinary team delivering high-quality, patient-centred care to frail patients, those requiring palliative support, and individuals approaching end of life.

The successful candidate will play a key role in supporting patients to remain safely within their own homes, preventing unnecessary hospital admissions, and delivering holistic care tailored to individual needs.

Full or part time hours considered.

Pay depending on experience.

Full UK driving licence and use of own vehicle is essential for this role.

Main duties of the job

  • Provide advanced clinical assessment and management within the community setting
  • Deliver high-quality care to frail patients and those with complex long-term conditions
  • Support patients requiring palliative and end of life care
  • Work closely with GPs, community teams, care homes, hospices, and wider multidisciplinary services
  • Undertake autonomous clinical decision-making and care planning
  • Contribute to service development and integrated working across the PCN

Job responsibilities

Clinical Responsibilities

  • Undertake advanced clinical assessments, history taking, and physical examinations within the community setting.
  • Develop, implement, and evaluate individualised care plans for patients with complex health needs.
  • Provide care and support for frail patients, including anticipatory and proactive care management.
  • Deliver high-quality palliative and end of life care in line with patient wishes and best practice guidance.
  • Identify deteriorating patients and initiate appropriate interventions and escalation where required.
  • Support admission avoidance and facilitate early supported discharge where appropriate.
  • Work autonomously within professional scope of practice and clinical competence.
  • Maintain accurate and timely clinical records in accordance with organisational and professional standards.

Leadership and Team Working

  • Work collaboratively within the multidisciplinary team including GPs, community nurses, allied health professionals, social care, hospices, and voluntary services.
  • Provide clinical leadership and support to junior staff where appropriate.
  • Contribute to service improvement initiatives and integrated working across the PCN.
  • Participate in team meetings, case discussions, and clinical supervision.

Communication

  • Communicate effectively and sensitively with patients, relatives, carers, and professionals.
  • Support difficult conversations relating to frailty, deterioration, advance care planning, and end of life care.

Professional Responsibilities

  • Maintain professional registration and comply with the NMC Code of Conduct.
  • Participate in continuing professional development and mandatory training.

Ensure safeguarding responsibilities are upheld for vulnerable adults

Person Specification

Experience

Essential

  • Extensive post-registration nursing experience.
  • Experience in community, primary care or integrated care settings.
  • Proven autonomous management of complex caseloads.
  • Leading or contributing to MDT's.
  • Involvement in service development or quality improvement.
  • Experience working with frailty or long-term conditions.

Desirable

  • Experience in cancer and palliative care.
  • Participation in clinical audit/research.

Knowledge and Skills

Essential

  • Good teamworking skills.
  • Excellent verbal and written communication.
  • Sound knowledge of clinical governance, safeguarding and professional regulation.
  • Knowledge of integrated care models and population health principles.
  • Coaching, mentorship or teaching skills.
  • Understanding of NHS priorities, policies and community service.
  • IT proficiency, including electronic patients records and digital care tools.
  • Compassionate, patient-centred approach.
  • Commitment to holistic care across diverse populations.
  • Ability to work independently and manage competing priorities.
  • Flexible and responsive to service demands.
  • Collaborative mindset with a focus on integrated joined-up care.
  • Culturally aware and committed to equality and inclusion.

Desirable

  • Knowledge of palliative care pathways and end-of-life care planning.

Qualifications

Essential

  • Registered Nurse (NMC)
  • Post-Graduate level physical examination module

Desirable

  • Teaching/Mentorship qualification (e.g. Practice Assessor)

Essential

  • Possess a valid UK driving licence with access to use of own vehicle
