Job summary
We are seeking a compassionate, proactive, and motivated individual to join our team as a Social Prescriber / Health & Wellbeing Coach. This dual role offers an exciting opportunity to support patients in improving their physical, mental, and social wellbeing through personalised coaching, behaviour change support, and connection to community-based services and resources.
Working closely with the practice, multidisciplinary teams, and voluntary sector organisations, you will empower individuals to take greater control of their health and wellbeing, helping them to identify and achieve meaningful goals while addressing wider determinants of health.
Main duties of the job
Key Responsibilities
Social Prescribing
- Conduct person-centred conversations to identify individual needs, strengths, and aspirations.
- Support patients experiencing social, emotional, practical, or non-medical challenges affecting their health and wellbeing.
- Develop personalised support plans and connect individuals to relevant community groups, activities, and services.
- Build and maintain strong relationships with local voluntary, community, and statutory organisations.
- Advocate for patients where appropriate and support them to access services independently.
- Maintain accurate records and demonstrate outcomes using agreed reporting systems.
Health & Wellbeing Coaching
- Deliver personalised health coaching interventions to support behaviour change and self-management.
- Support patients with long-term conditions to improve confidence, knowledge, and self-care.
- Use evidence-based coaching techniques including motivational interviewing and goal setting.
- Work collaboratively with patients to identify barriers to change and develop achievable action plans.
- Promote healthy lifestyles and support improvements in areas such as physical activity, nutrition, smoking cessation, stress management, and overall wellbeing.
- Contribute to reducing health inequalities and improving patient outcomes.
About us
Whilst we work extremely hard, the culture of the practice is friendly and welcoming. Prior to retirement, several members worked at Riverside Medical Practice for over 15 years. We have an informal meeting room where team members often enjoy lunch together.
The GP team meet daily for an informal coffee catch up/peer support.
The team comprises of:
Two GP Partners, two salaried GPs, two Advanced Care Practitioners, two paramedic practitioners providing acute appointments, Clinical Pharmacist, three Practice Nurses, a Health Care Assistant and a Nurse Associate.
The practice management team compromises of the Practice Manager who has worked at the practice for over 40 years and a Deputy Manager.
The support team comprises of 14 clerical members of staff.
Job responsibilities
The Social Prescriber / Health & Wellbeing Coach will work collaboratively with patients, GP practice, and community organisations to support individuals in improving their health, wellbeing, and quality of life. The postholder will provide personalised coaching and social prescribing interventions, helping people identify and achieve their health goals, build confidence in managing their own wellbeing, and access appropriate community-based support and services. The role requires a person-centred approach, strong communication skills, and the ability to empower individuals to make positive and sustainable lifestyle changes. Healthcare Assistant (HCA) qualifications and experience are advantageous and may be utilised to support holistic patient care where appropriate.

- Qualified to NVQ level 2 in Health and Social Care
Experience

- Experience working within healthcare, community, wellbeing, social care, or voluntary sector settings.
- Excellent communication and interpersonal skills.
- Ability to build trusting relationships with individuals from diverse backgrounds.
- Understanding of person-centred care and behaviour change approaches.
- Experience supporting vulnerable individuals and managing complex needs.
- Ability to work independently and as part of a multidisciplinary team.Strong organisational, IT, and record-keeping skills.
Desirable
- Social Prescribing Link Worker training or relevant experience.
- Healthcare Assistant (HCA) qualification and/or experience in a clinical setting.
- Knowledge of local community resources and support services.
- Experience working within Primary Care, NHS, or Primary Care Networks.
- Training in motivational interviewing, health coaching, or behaviour change methodologies.
