Job summary
We are thrilled to announce an excellent opportunity for an experienced Social Prescriber to undertake holistic assessments and co-design Health and Well-being Plans with individual patients, identifying support needs to ensure maximum engagement in improving health and well-being, to join our team at West Moors Village Surgery under the Additional Role Reimbursement Scheme (ARRS).
Main duties of the job
The Social Prescriber supports individuals to improve their health, wellbeing, and social connectedness by helping them access community services, activities, and support.
Working closely with GP practices, healthcare professionals, voluntary organisations, and community groups, the Social Prescriber empowers people to take greater control of their health and address non-medical factors affecting their wellbeing.

We are a friendly practice situated in the heart of West Moors, in the beautiful area of East Dorset, known for its heathlands and woodlands.West Moors Village Surgery serves just over 5,400 patients from our surgery at Vernon House, 164 Station Road, aiming to provide traditional patient-centred care for people of all ages.
Job responsibilities
To undertake holistic assessments and co-design Health and Well-being Plans with individual patients, identifying support needs to ensure maximum engagement in improving health and well-being.
To work with patients, using behaviour techniques such as motivational interviewing and guided conversations to support them to identify, set and meet achievable goals to improve their physical and emotional wellbeing.
To provide patients with continuity and a coordinated experience of care and remaining the point of contact throughout the individuals social prescription.
To work with patients to signpost them to local services / organisations which the patient themselves feel would be of benefit to reach their goals. These services may include, befriending, weight management, mental health support, smoking cessation, local social groups, leisure centres and volunteering etc. to name but a few.
To follow up with patients, update their records and support them on an ongoing basis.
To ensure information on sources of voluntary and community support is up to date at all times to enable effective and accurate signposting and linking of individuals with services.
To undertake regular training to extend and update own knowledge of health and wellbeing issues.
To establish and maintain effective liaison with stakeholders including health, voluntary, social and education resources, attending relevant meetings as necessary.
To train and develop GPs and primary care health teams knowledge on how to identify patients suitable for social prescribing service referral and to keep the service at the forefront of clinicians minds by means of being present at MDTs, social media, regular updates, practice meetings etc. Work closely with the Practice Manager to ensure their understanding of the role and enable them to disseminate accurate information to their teams.
Maintain a SystmOne template to ensure accurate and detailed clinical data is entered into the patient record in a standardised way. To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required.
Provide comprehensive outcome focused reports detailing the progress of the service using information from the template above where required.
To ensure you have an understanding (appropriate to your role) of, and comply with procedures for promoting and safeguarding the welfare of children and vulnerable adults.

Qualifications
Essential
- High standard of education (GCSE or equivalent in English & Maths)
Desirable
- Relevant vocational qualification, HEE e-learning programme & Personalised Care Institute (PCI)
- Evidence of continued professional development
Experience
Essential
- Experience of working in a primary care environment
- Experience of partnership working and building relationships with a range of stakeholders
Desirable
- Experience of working with individuals to identify needs and develop personalised support plans
- Experience of working in a healthcare setting
