Are you passionate about supporting older adults to live safely, independently, and with dignity? We are looking for committed social workers to join our dedicated Older Peoples Team.

Main duties of the job

As Social Worker within the Older Peoples Community Team, youll assess the needs of people aged 65+ and develop personalised care plans that promote independence, wellbeing and quality of life. Youll work closely with individuals, families and carers, using strengthbased approaches to support people to stay connected to their communities. Youll work collaboratively across a wide range of services, including hospitals, community teams and partner organisations, helping ensure smooth and effective support for those in your care. Youll also contribute to service development and support the delivery of highquality social work practice. Working as part of a supportive and dedicated team, youll play a key role in improving outcomes for older people across the Island. Essential

  • Social Work Professional Qualification (e.g. BA/MA Post Graduate Diploma in Social Work, Dip. SW, CQSW).
  • Be registered with Social Work England.
