Job summary
The Role
We are looking for an experienced, compassionate and motivated Staff Nurse to join our Hospice at Home team, based at Rowans Hospice in Purbrook.
This rewarding role offers the opportunity to deliver specialist palliative and end-of-life care to patients in the comfort of their own homes. Working both independently and as part of a multidisciplinary team, you will provide high-quality, person-centred nursing care while supporting patients and their families through some of lifes most challenging times.
You will contribute to the assessment, planning, implementation and evaluation of individualised care programmes and, where appropriate, provide guidance and support to more junior nursing colleagues.
Main duties of the job
Key Responsibilities
Your responsibilities will include, but are not limited to:
- Assess patients needs and contribute to the development, implementation and evaluation of individual care plans delivered within patients homes.
- Build and maintain compassionate, professional relationships with patients, their families and carers, ensuring care is tailored to their individual needs.
- Deliver evidence-based nursing care, undertaking clinical procedures and treatments in line with current best practice.
- Assess when input from other healthcare professionals or specialist services is required and make appropriate referrals, seeking guidance from senior colleagues where necessary.
- Communicate effectively with GPs, District Nurses, Hospice Doctors, the Hospice at Home Co-ordinator and Senior Hospice Nurses to ensure continuity of care.
- Participate in the out-of-hours service, reporting patient concerns and operational issues to the appropriate on-call professionals.
- Verify expected deaths where appropriate, following organisational policies and procedures, and provide sensitive support to families and carers.
- Carry out last offices and ensure the safe and dignified transfer of deceased patients in accordance with community procedures.
- Administer, order and manage medicines in accordance with the NMC Standards for Medicines Management, reporting any medication errors or concerns appropriately.

At Rowans Hospice, we are a local charity providing free specialist palliative and end-of-life care to adults living with life-limiting illnesses across Portsmouth and South East Hampshire. We also provide invaluable support to their families and loved ones.
Since 1994, our services have been made possible through the generosity of our local community, with funding coming from donations, gifts in Wills, fundraising activities, and income generated through our retail shops.

- Excellent basic nursing care skills
- Good communication and interpersonal skills
- Ability to prioritise work load and be flexible to changing demands
- Ability to share knowledge with others to develop and influence practice
- Able to maintain active and contemporaneous records
- Awareness of own coping strategies
Desirable
- Basic understanding of palliative care principles and philosophy
Qualifications
Essential
- RGN or RN Adult Branch
- On NMC Register parts 1 or 2
- Experience in palliative care
- 1 years post registration experience
- Driving License/Car Owner
Desirable
- Community experience
- Post registration qualification
