Harrold Medical Practice is seeking an enthusiastic, motivated and patient focused Advanced Nurse Practitioner (ANP) who is adaptable, thrives on team work and can work effectively within our large friendly and supportive team.

We are dedicated to delivering high quality, compassionate care to our 6,750+ patients.

This is an exciting opportunity for an experienced ANP to work within a forward thinking and innovative Practice. The successful candidate will work collaboratively within the nursing and GP team to deliver safe, effective and patient centred care.

Main duties of the job

The Advanced Nurse Practitioner (ANP) will work autonomously and collaboratively within the nursing team to deliver safe, effective and evidence-based care to the patients of Harrold Medical Practice. The post holder will undertake advanced clinical assessment, diagnosis, treatment, prescribing referral and ongoing management of patients presenting with acute, chronic, complex and undifferentiated conditions.

Job responsibilities

The ANP will demonstrate expert clinical judgement, autonomous decision-making and advanced clinical skills.

Responsibilities include: Advanced Clinical Assessment and Treatment

Undertake comprehensive history taking, clinical assessment, examination and diagnosis.

Assess and manage patients presenting with acute, chronic, minor illness and complex conditions.

Independently formulate treatment plans and deliver evidence-based interventions.

Prescribe safely and effectively within scope of practice and prescribing qualification.

Request, interpret and act on diagnostic investigations and test results.

Identify deterioration and manage clinical risk appropriately.

Refer patients to secondary care, community services and allied professionals where required.

Deliver same-day urgent care and telephone triage services.

Nursing Procedures and Long-Term Condition Management

Deliver basic and advanced nursing interventions.

Perform wound care and management.

Undertake venepuncture and ECG recording.

Administer vaccinations and immunisations in accordance with national guidance.

Undertake cervical cytology where trained.

Deliver family planning and sexual health services where competent.

Support chronic disease management including asthma, diabetes, cardiovascular disease and respiratory conditions.

Support achievement of Quality and Outcomes Framework (QOF) objectives.

Operational Clinical Responsibilities

Ensure accurate and timely recording of clinical data within practice clinical systems.

Maintain familiarity with GMS/PMS contractual requirements.

Ensure safe storage and management of medicines.

Adhere to treatment room protocols and infection prevention standards.

Act as a chaperone in accordance with Practice policy.

Maintain patient dignity, confidentiality and person-centred care.

Service requirements may evolve to meet operational demands including seasonal vaccination programmes and service redesign.

Leadership and Management

The ANP will contribute to the delivery of high-quality services.

Responsibilities include:

Act as a clinical role model and advocate for advanced nursing practice.

Support development and implementation of practice objectives.

Participate in service redesign and continuous improvement initiatives.

Lead and support clinical governance, patient safety and risk management.

Contribute to audit and quality improvement programmes.

Support operational effectiveness and workforce planning.

Promote equality, diversity and inclusion.

Support implementation of local and national healthcare priorities.

Education

The ANP will support learning, professional development and knowledge sharing.

Responsibilities include:

Deliver patient education and self-management support.

Participate in teaching and educational activities.

Maintain personal Continuing Professional Development (CPD).

Participate in appraisal and professional revalidation processes.

Maintain competence and develop specialist clinical skills

Research

The ANP will contribute to evidence-based practice and service improvement.

Responsibilities include:

Apply current evidence and clinical guidance to practice.

Participate in clinical audit and service evaluation.

Support quality improvement initiatives.

Monitor and evaluate patient outcomes.

Contribute to practice data collection and reporting.

Support implementation of research findings into practice.

Maintain awareness of developments in advanced clinical practice.

Accountability and Professional Practice

Practise in accordance with the Nursing and Midwifery Council Code and maintain current registration.

Work within professional competence and scope of practice.

Remain accountable for all clinical decisions and actions.

Seek support and escalate concerns appropriately.

Comply with Information Governance, Health and Safety, Infection Prevention and Control and Practice policies.

Maintain professional standards and contribute positively to team culture.

Qualifications

Essential

  • Registered General Nurses with NMC registration.
  • Master's level qualification in Advance Clinical Practice/Advanced Nurse Practitioner.
  • Independent Prescriber qualification.
  • Evidence of ongoing CPD and professional development.
  • Advanced consultation and diagnostic skills.
  • Excellent communication and interpersonal skills.
  • Sound clinical judgement and decision-making ability.
  • Strong organisational and leadership capability.
  • Competence using clinical systems and maintaining accurate records.
  • Ability to manage complexity and uncertainty.

Desirable

  • Understanding of population health and preventative care.
