Job summary

We are looking for a Learning Disability Liaison Nurse to lead and support high-quality, person-centred care for people with learning disabilities across hospital, primary and acute services on the beautiful island of Jersey.

The role involves working closely with multidisciplinary teams to improve patient experience, ensure reasonable adjustments are in place, and promote seamless care between services. You will provide specialist advice, education and direct support to staff, patients and families, helping to reduce health inequalities and ensure safe, effective care throughout the patient journey. All within access to beautiful beaches, costal walks, and access to numerous outdoor activities in a safe and peaceful environment.

Main duties of the job

Provide specialist LD nursing liaison across acute, primary and urgent care pathways so that people with a learning disability have equitable, person centred access to assessment, treatment and safe discharge. Lead the identification, coordination and documentation of reasonable adjustments, advocate for patients and carers, and support clinical teams to meet legal and professional standards including the Capacity and Self Determination Jersey Law 2016 and its Code of Practice.

Qualifications

Essential

  • 1st Level Registered Nurse
  • RNLD RNMH RNID
  • Teaching Mentorship
  • Qualification or equivalent practice
  • assessor/supervisor preparation
  • Degree level professional qualification with
  • minimum 2 years post registration experience,
  • including recent practice in a relevant specialty.

Desirable

  • 2 years post qualifying
  • experience including 1
  • years recent experience in
  • specialty area
  • Relevant post qualifying
  • speciality award or evidence
  • of professional development
  • Working towards 120 credits
  • at Level 7
  • Additional training in areas
  • such as mental capacity,
  • safeguarding, or leadership
