Job summary
We are seeking an experienced and motivated INT Transformation Manager to lead the development, delivery and evaluation of Integrated Neighbourhood Teams (INTs) across Nottingham City.
Working closely with clinical, operational, community and system partners, you will support the roll-out of this major transformation programme, building on the Pathfinder Blueprint developed in Nottingham City East and Bestwood & Sherwood.
This is a pivotal role focused on shaping service redesign around local needs and priorities while ensuring strong programme governance, impact measurement and clear reporting of outcomes.
You will provide senior transformation, project and programme management expertise to drive integrated working, service improvement and sustainable change across CityCare and the wider Nottingham City system.
Main duties of the job
- Lead and coordinate transformation activity for Integrated Neighbourhood Teams across Nottingham City.
- Provide expert programme and project management support, including planning, governance, risk management and benefits realisation.
- Work collaboratively with clinicians, service leads, commissioners, local authority partners, primary care, voluntary sector organisations and community stakeholders to design and mobilise new models of care.
- Build and manage effective stakeholder relationships across the system to support delivery, engagement and long-term sustainability.
- Prepare high-quality reports, presentations and briefings to support decision-making, performance monitoring and approval processes.
- Facilitate workshops, planning sessions and collaborative forums to support co-design and implementation.
- Act as an ambassador for CityCare, promoting partnership working, organisational values and a shared vision for neighbourhood-based care.

Job Purpose
- To work closely with clinical, operational, community, and system stakeholders to lead the development, delivery, and evaluation for the roll out of Integrated Neighbourhood Teams (INTs) across Nottingham City.
- This transformation will be based on the Pathfinder Blueprint from Nottingham City East and Bestwood & Sherwood INTs. This role will ensure local needs and priorities help shape service redesign, while maintaining robust arrangements for programme governance, impact measurement, and reporting of outcomes.
- The role provides senior transformation project and programme management expertise to support integrated working, service improvement, and sustainable change across CityCare and the wider Nottingham City system.
Dimensions
- The role will provide leadership and programme delivery support for integrated neighbourhood team transformation across Nottingham City, working closely with senior leaders from across the system in a virtual partnership team.
- The post will work collaboratively with service leads, clinicians, commissioners, local authority partners, primary care, voluntary sector organisations, and community stakeholders to support the design, mobilisation, and delivery of integrated neighbourhood teams.
- The role will influence and manage key stakeholder relationships and contribute to strategic planning, performance improvement, and the development of future service models aligned to system priorities.
- The role has no direct budgetary responsibility.
Key Responsibilities
- Lead and coordinate transformation and service development activity across integrated neighbourhood teams, including identifying priorities, developing plans, and monitoring delivery.
- Provide expert project and programme management support for complex transformation workstreams, ensuring clear governance, milestones, risk management, and benefits realisation.
- Work with clinical and operational leaders to support this neighbourhood service redesign and integration across health, social care, and community services.
- Develop and maintain effective working relationships with commissioners, provider partners, local authorities, primary care networks, and voluntary and community sector partners.
- Facilitate workshops, planning sessions, and collaborative forums to support co-design, engagement, and implementation of change.
- Prepare high-quality reports, briefings, presentations, and other documentation to support decision-making and approval processes.
- Analyse and interpret complex information from multiple sources to inform recommendations, planning, and performance monitoring.
- Identify, escalate, and manage programme risks, issues, dependencies, and barriers to delivery.
- Support the development of policies, protocols, and guidance required to implement new ways of working.
- Provide leadership, support, and line management responsibilities where required, including staff development, performance review, and workload prioritisation.
- Act as an ambassador for CityCare, promoting organisational values and collaborative working across the wider system.
- Ensure effective communication with a wide range of partners and key stakeholders that ensure a high degree of visibility and understanding of the programme of transformation.
- Make formal presentations to large groups, including senior managers both internally and from partner organisations, providing specialist knowledge on the specific workstreams as well as the programme as a whole.
- Provide and receive complex, sensitive, or contentious information to small and larger groups of internal and external stakeholders.
- Display tact and diplomacy when communicating with internal and external stakeholders around areas which may require negotiation, building understand or be contentious in nature.
- Build mutually beneficial relationships with a range of key internal and external stakeholders, demonstrating organisational values and acting as an ambassador for CityCare.
- Communication and engagement with external stakeholders, building positive working relationships with local and regional system partners, other provider organisations and third sector partners.

Experience
Essential
- Significant experience of leading and delivering complex transformation or service improvement programmes in the NHS or public sector; experience of partnership working across organisational boundaries; experience of developing business cases, project plans and service redesign proposals; experience of engaging clinical and operational leaders in change programmes.
Desirable
- Experience of neighbourhood working, integrated care, or community health service redesign.
Knowledge and skills
Essential
- Knowledge of current NHS policy, integrated care and neighbourhood working; strong analytical, written, presentation and facilitation skills; ability to manage multiple priorities, influence stakeholders and solve problems effectively; understanding of governance, risk management and benefits realisation.

- Educated to masters level or equivalent experience; evidence of continuing professional development; project or programme management qualification or equivalent experience.
