Establish and maintain excellent working relationships with statutory, voluntary and community groups to promote existing support for carers. Develop partnership work in local market towns to initiate new forms of support for carers and ensure that the existing provision is accessible and open to carers. Provide support and information to carers and their families through local drop-ins, peer support opportunities, telephone, home based contact and other digital means according to need. Offer assessments or reassessments to carers in line with the Care Act 2014. To undertake any other duties that may be considered commensurate with the level of the post. In-house training and development opportunities will be available to meet identified training needs essential for the post. Who will your manager be and what will you be responsible for? You will be: • reporting to the Shropshire Carers manager who is in turn responsible for your health and safety, training and development. • expected to lead and/or contribute to a range of projects and build relations with both internal and external partners in order achieve specified outcomes but will not have direct authority over those involved. Increasing local community resilience through carers coming together as peer groups for support; develop groups and networks to form alternative forms of respite provision in local communities and organising and/or linking carers to local activities to improve carer resilience and well-being. • • Facilitate and support the growth of peer groups for a time limited period; work with individuals, groups and other key stakeholders to ensure future sustainability • Building partnerships with Adult Social Care teams, GP C&CC's and other statutory, voluntary and community groups to develop new initiatives to improve support for carers. • Alongside the Carer team Co-ordinator, recruit and support locally based volunteers, including current or former carers, to offer support, information and help to carers through befriending, at the peer groups and through telephone or online contact. • Provide an assessment of need and devise and implement an appropriate care plan to achieve improved outcomes for carers. Offer information regarding available services, including voluntary and community services; provide 1:1 support and advice to carers through home visits, digital options and telephone contact. • Support carers to actively plan for the future and provide a personalised future plan to enable carers to manage changes in their caring role. • The service and support provided is person centred and aims to support carers to achieve their personal outcomes. • Assist with event planning, set up and promotion. What will you be doing? The post holder will: • Hold a caseload with appropriate supervision and advice. • Work positively and productively with Carers, teams within the Council and its partners and any other relevant providers, developing a network in your designated area to build the Shropshire Carers partnership. • Contribute to the development of new models of work and service under the supervision of the Carers Lead. • Participate in meetings which will include team, peer sessions, case review meetings etc. • Engage with statutory, voluntary and community sectors, including the DWP/Job Centres, to raise awareness of carers and provide information to enable them to improve workplace support for carers. • Provide a first point of contact for carers by being part of the ongoing support line rota. • Maintain and enter records of activities in accordance with approved Policies and Procedures, including the use of a computer system database (i.e. Liquid Logic) • Comply with safe systems of work in accordance with the Directorate's Health, Safety and Welfare Policies and Procedures. • Ensure your individual and service performance targets are met. • Respect the personal choice and lifestyles of colleagues, carers, and people with care needs, ensuring that equal opportunity principles are always applied. • Have responsibility for promoting continuing personal and professional / vocational development of self through development processes. • Be prepared to participate in such other appropriate duties as may be allocated from time to time by the Carers Lead. • Ensure engagement with carers and service users will be non-discriminatory, listen and respect their views and opinions and apply a customer focused approach when delivering the service. • Act as advocate for Shropshire Carers and work collaboratively with colleagues across the whole Council to meet the needs of the people of Shropshire. Your primary work base will be at The Guildhall, Shrewsbury with the requirement to regularly work from home. Your post is subject to the following: ▪ The post is Permanent for 37 hours per week, ▪ Normal office hours are 7am - 7pm Monday - Friday and 8am - 1pm Saturday with a minimum lunch break of 30 minutes. Occasional evening work may be required, ▪ The post is subject to the Council's annualised hours scheme as implemented within the specific work area. Annual leave entitlement is a pro rata flat rate scheme of 207 hours (28 days) annual leave plus bank holidays, with five days extra awarded to those staff with five years local government service giving an entitlement of 244 hours (33 days). Two days of an employee's allowance (pro rota for part time staff) must be taken at Christmas for any potential Christmas closures. Employees who work in a building/service which is required to open over the Christmas period, the two days leave (pro rota) can be carried over into your next leave year but must be used by the end of March. The appointment is subject to one months' notice in writing on either side. The appointment is subject to six months' satisfactory probationary service during which time the notice period will be one week on either side. Smoking is not allowed in Council buildings, in Council vehicles or in any Council place of work. It is a condition of your appointment that you provide a suitable vehicle for the performance of your duties and that this is readily available for use during normal working hours.
