Contract: Fixed term to end March 2026 with the possibility of an extension
Salary £21,840 - £22,781 per annum based on 35 hours per week

What will a day in the life of a Service Coordinator involve?
- Meeting with our service users and assessing their needs, guiding or signposting them to connect with groups or to manage a health condition. You'll enable them to lead independent lives and improve their wellbeing.
- Liaising with partnership agencies to promote the service and build meaningful links.
- Producing and maintaining monthly reports.
What do you need to be a successful Service Coordinator?
- Good communication skills.
- Sound knowledge of Health and Social Care services.
- Proven ability to deliver high quality public services.
- A manual UK driving licence, held for minimum of two years, and access to a vehicle (business mileage will be paid).
