This is a great opportunity for an experienced social worker or a Senior Practitioner who would like a different challenge to join the Chesterfield Royal Hospital Discharge team. Reporting to the Service Manager, your role will be to supervise social care staff and lead on professional practice. You will be involved with the day to day running of the team. You will support Adult Social Care colleagues both within the local team and on a countywide level including the identification of training and development needs through Peer Supervision.

As a senior practitioner you will lead by example and ensure that Social Workers and Social Care Practitioners deliver high quality statutory social care services to adults over the age of 18 who need to leave hospital in a timely manner. You will be actively involved in complex case management and court work if required. You will offer support, advice, and professional supervision to all members of the team. You will produce management reports, data reports, chair meetings, and deputise for the Service Manager when required.

As this role involves multi professional and multi-agency working, you will need to be able to demonstrate strong leadership abilities to provide guidance, direction, and mentorship to staff, foster a positive work environment, and promote a culture of continuous improvement.

The successful candidate will be able to work autonomously and will have excellent knowledge of Safeguarding, The Care Act 2014, and the Mental Capacity Act 2005.

You will also be responsible for mentoring and supporting students, ASYE and hold Practice Educator qualifications or express an interest in completing the relevant training to undertake this task.

This is a full time, 5 in 7 post although currently the service is run Monday - Friday, there may be a requirement for 7-day service in the future.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.

Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
