Job Description
We are looking to recruit to a permanent Senior Practitioner post within the Short-Term Assessment and reablement team in South Derbyshire.
We welcome applicants with a Social Work or Occupational Therapy qualification and with experience of working in an Adult Social Work setting. To be an effective communicator and an ability to lead the development of an integrated approach. To be able to demonstrate an ability to work with a person centred and strength-based approach to achieve the ideal outcomes with people to live as independent as possible within their own homes and communities is essential. Experience of a collaborative approach to multidisciplinary working is required to support people to return home or remain in their homes following a period of ill health or change in needs.
The successful candidate will need to have a knowledge of the current legislation, policy and procedures that underpin good practice and the application of quality assurance. An understanding and experience of Safeguarding principles and practice will be essential. It essential that the successful candidate can support the learning and development of workers at all levels within the team.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.
Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
