Job Description

We are looking to recruit a qualified Social Worker to join our Prison Social Work Team based in South Derbyshire. There is the expectation that the successful applicant will also undertake case work within the area social work team. The successful applicant will be able to demonstrate the necessary skills, knowledge and experience to undertake this role to provide safe, accountable, cost effective and efficient services to our clients.

The role includes carrying out Care Act (2014) assessments, support plans and reviews for individuals with complex needs as well as arranging the provision of services to meet eligible needs.

The role also includes:-

  • Assessments under section 76 of The Care Act 2014 for both support in custody, parole and release planning
  • Review care provision and liaise with provider.
  • Attending safety meetings
  • Attending MAPPA
  • Attend perinatal meetings.
  • Be part of the MDT and attend regular meetings.
  • Attend CPA and 117 reviews.
  • Liaise with community social work teams and support visits.
  • Support Occupational therapy visits and ordering of relevant equipment.
  • Duty worker triaging new referrals and support those in need of an urgent intervention.
  • This role requires additional clearance for any successful applicant through HMP services.

The role is hybrid working with an expectation of regular travel to the prisons.
