Are you passionate about making a difference in the lives of adults in your community? Lancashire County Council is seeking a dedicated and professional Social Worker to join our Adult Social Care team. This role involves providing a strength-based, outcome-focused assessment service across community and hospital settings.
Key Responsibilities:
- Deliver a professional social work service in line with statutory responsibilities and organizational policies.
- Promote independence, enable recovery, and safeguard adults through strength-based and outcome-focused service delivery.
- Collaborate with other professionals and agencies to address complex needs and achieve optimal outcomes for individuals and their carers.
- Conduct high-quality assessments and develop support plans in consultation with individuals and their carers.
- Prepare and present written reports effectively in various formats.
- Work with health colleagues to ensure safe and timely discharges from hospital settings.
- Promote the creative and flexible use of personal budgets to meet the needs of those we support.
- Communicate effectively with people, carers, other agencies, and professionals.
- Manage and prioritize a caseload efficiently.
- Maintain accurate records using electronic systems.
- Contribute to team working and service development.
- Support and mentor newly qualified or less experienced staff and students.
- Engage in continuous professional development and mandatory training.
- Maintain emotional resilience and professional wellbeing.
- Participate in a duty system and respond to urgent situations as needed.
Experience:
- Accepting referrals and gathering information.
- Assessing the needs of individuals and their families.
- Managing a caseload and determining priorities.
- Working effectively as part of a team.
- Experience in a Social Services Directorate or other statutory or voluntary agency (Desirable).
Skills and Abilities:
- Knowledge of relevant social work legislation.
- Ability to assess and analyze needs and risks, and plan care with individuals.
- Effective collaboration with other agencies and professionals.
- IT literate with experience in record-keeping systems.
- Strong organizational skills and ability to work independently under pressure.
- Excellent written and oral communication skills.

You must be registered (SWE) to be appointed as a Social Worker or have applied to register with the Social Care Council. For Social Worker/Care Manager positions where non social worker qualifications are accepted, registration is not required, and you will be appointed as a Care Manager.
