We are looking for a compassionate and dedicated Social Care Support Officer to join our Community north team. The successful candidate will provide short-term social care interventions, personalized to the individual's care and support needs, using a strength-based approach. This role aims to empower individuals, build their confidence, and enhance their social networks.
Key Responsibilities:
- Strength-Based Service: Provide a strength-based, outcome-focused service to all adults across community and hospital settings using the 3 Conversations approach.
- Collaborative Work: Work with other professionals and agencies to address needs, encourage independence, enable recovery, and safeguard adults.
- Community Engagement: Support individuals to participate in community activities, reduce social isolation, and develop local community networks.
- Economic Well-Being: Ensure access to appropriate benefit entitlements and link individuals with debt advice agencies.
- Promote Independence: Support individuals in daily activities, build their confidence, and address personal anxieties.
- Mental Health Awareness: Link with local services and agencies to promote mental health awareness and the use of public transport.
- Digital Technology: Promote the use of digital technology to improve mental well-being.
- Person-Centred Planning: Co-produce well-being plans with individuals using coaching and motivational interviewing techniques.
- Workload Management: Effectively prioritize and manage your workload with support from your line manager.
Person Specification:
- Qualifications: GCSE Grade 'C' or equivalent in four subjects including Maths and English; Basic qualification in Social Care (desirable).
- Experience: Experience in an organization providing care/health services and assessing individual needs for services.
- Skills and Abilities: Effective assessment skills, knowledge of support plans, good communication skills, ability to work independently and within a team, IT literacy, and knowledge of local resources.
- Other Requirements: Commitment to equality and diversity, health and safety, and the ability to drive and have a car at your disposal.
