Lancashire County Council is seeking a dedicated and compassionate Social Worker to join our dynamic ICAT team at CUCAP Health in Skelmersdale - West Lancashire. Key Responsibilities:

  • Provide comprehensive social work services to individuals in urgent care, acute, and prison environments.
  • Collaborate with healthcare professionals to develop and implement care plans.
  • Advocate for the needs and rights of service users.
  • Conduct assessments and provide crisis intervention as needed.
  • Maintain accurate and timely documentation.

Working with a range of professionals in the Short-Term Support Service Team, you will:

  • Work with people being discharged from hospital or avoid an unnecessary admission to hospital or a care home and ensure that people have the care and support they need to recover.
  • Carry out high-quality face-to-face Care Act Assessments and reviews.
  • Develop and review strengths-based support plans including therapeutic goals.
  • Using a strengths-based approach, ensure that people receive the right care and support at the right time, enabling them to achieve their optimum level of independence and live their life in a way that they choose.
