We are looking for a highly skilled Advanced Clinical Practitioner (ACP) to join our dynamic team, providing exceptional patient-centred care.

Main duties of the job

As an ACP, you will work autonomously at an advanced level, using high-level decision-making skills to assess, diagnose, and treat patients across a wide range of primary and urgent care needs. Your role will include: Telephone triage & remote assessments Ensuring patients receive the right care, at the right time. Face-to-face clinical assessment & diagnosis Delivering expert care for undifferentiated and urgent conditions. Management of all general practice needs Including paediatrics, elderly care, and mental health. Independent prescribing Working within current prescribing legislation to optimise patient treatment plans.
