Advanced Diabetes Dietitian (Transition)

Job responsibilities

Autonomously provide expert, holistic, patient-centred dietetic care to individuals with diabetes, particularly the transition age diabetes population, underpinned by clinical, professional and Department guidelines and integrating with other services.

Provide following specialist diabetes skills:

o Practical knowledge and skills required for insulin pump therapy for people with Type 1 diabetes

o Education to people with diabetes to enable them to adjust their insulin dosages and carbohydrate in order to achieve their targets for blood glucose control.

o Education regarding technology and data interpretation for example continuous blood glucose monitoring.

o Identification of appropriate treatment aims for people with type 2 diabetes using blood glucose-lowering agents and explain their mechanisms of action. Working with the multidisciplinary team to make suggestions to the suitable glucose-lowering agents.

Provide the transition service and if required, contribute to the cover of:

o Secondary care clinics and outreach

oBERTIE and Living With Diabetes (for newly diagnosed type 1 diabetes) group education.

o Antenatal clinics

o Inpatient care at Dorset County Hospital.

oIndirect patient care via support to secondary care staff e.g. MDT meetings, Advice & Guidance enquiries, support to other DCH dietitians.

Be an integral part of the wider diabetes and dietetic teams within DCHFT and where competent to do so, support other services offered by the teams as and when the need arises.

Aid service resilience and skill maintenance by exchanging cover with the other diabetes dietitians as needed.

Actively manage the risks and benefits of own interventions within the MDT context.

Monitor and evaluate the clinical effectiveness of the specialist diabetes dietetics service to ensure quality and justify change.

Maintain and progress own knowledge and skills to ensure high quality and holistic dietetic care to patients with diabetes, who may have a number of comorbidities.

Ensure patient involvement in the development of the service.

Participate in or lead MDT meetings and case conferences as required.

Education, Qualifications & Training

Essential

• BSc in Dietetics or equivalent
• Evidence of CPD relevant to clinical working and people management
• HCPC registration

Desirable

• BDA and diabetes sub-group membership
• Management training

Knowledge & Experience

Essential

• Post-registration experience including specific experience in the management of complex patients with type 1 and type 2 diabetes
• Experience of working with diabetes technologies including insulin pumps and continuous blood glucose monitoring
• Experience of developing and delivering training to healthcare professionals of all levels of experience
• Experience of initiating, evaluating and sustaining changes to service development
• Experience of team management and/or leadership
• Experience of working autonomously and collaboratively within an MDT
• Able to negotiate effectively
• Experience in audit and clinic governance issues
• Research experience
• Student supervision experience
