Job Description
Job summary
We are looking for an experienced nurse who, within their professional boundaries, will provide autonomous care for patients within our PCN led Urgent Care Centre using history-taking and clinical assessment skills leading to diagnosis, treatment and evaluation of care. The Advanced Nurse Practitioner will demonstrate safe clinical decision-making and a high level of independent clinical care including assessment, diagnostic and prescribing skills for both urgent and long-term care.
Main duties of the job
- Assess and treat patients, adult and children, presenting with undifferentiated and undiagnosed acute illness or long term conditions,
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies;
- Work with patients and their carers in order to support compliance with and adherence to prescribed treatments;
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions;
- Undertake telephone triage and consultations
- Clinically assess, diagnose, plan, implement and evaluate interventions and treatments for patients with potentially complex needs;
- To order diagnostic tests and initiate referrals as appropriate for patient needs;
- To admit directly to hospital in acute medical need, undertaking a comprehensive handover;
- To interpret findings or reports and take responsibility for following up your patients.
- To contribute to the development of practice policies and procedures;
- To assess the impact of policy implementation on care delivery;
- Recognise and work within own competence and professional code of conduct as regulated by the NMC;

Red & Green Practice is an establish training practice on the edge of the New Forest and part of Waterside PCN. We have a large MDT of GP'S, NP, PN, Physiotherapist, Pharmacist, pharmacy technicians, Health & Wellbeing coaches & Care Navigators.

Role Overview

Key Responsibilities

Qualifications, skills and experience
RGN1 with current NMC registration
MSc Advanced Nurse Practitioner
Independent Nurse Prescriber
A minimum of five years experience preferably in a Primary Care environment
Able to work as part of a team and autonomously
Excellent written and verbal communication skills
Good computer skills, specifically EMISWeb
Clean driving licence
