Job summary
An excellent opportunity has arisen at Pocklington Group Practice, a forward thinking, patient focussed, friendly organisation with c18,700 patients. We are looking to recruit a full-time, Advanced Nurse Practitioner (ANP)/Urgent Care Practitioner (UCP) to deliver high quality, on the day, urgent care to our ever growing patient population. You will be a dynamic, caring individual with excellent communication and interpersonal skills. You must be registered with the Nursing and Midwifery Council, hold a Master's level/postgraduate degree and be a qualified independent nurse prescriber.
Main duties of the job
To assess, diagnose and plan management/treatment plans for patients with undifferentiated, urgent care presentations, liaising with primary and secondary care to optimise care outcomes.
About us
The nursing team at Pocklington Group Practice numbers 12 in total, comprising of a friendly and professional group of PNs, HCAs and a Phlebotomist along with a Specialist Nurse in long term conditions and overseen by the Nurse Manager.
The whole Practice staff work together to provide high quality, safe, personalised care for our patients in an integrated and forward-thinking medical facility which supports staff and allows them to grow and reach their full potential.

Experience
Essential
- Experience of practice within the four pillars.
- Job plan that demonstrates advanced clinical practice and has equity with peers working at this level.
- Experience of prescribing and undertaking medication reviews.
Desirable
- Experience of working in a primary care environment.
Clinical Knowledge and Skills
Essential
- Clinical knowledge and skills to include urgent care.
- Understand the importance of evidence-based practice.
- Broad knowledge of clinical governance.
- Ability to record accurate clinical notes.
- Ability to work within own scope of practice and understand when to refer to GPs.
- Knowledge of health promotion strategies.
- Understand the requirement for PGDs and associated policy.
- Polite and confident, flexible and cooperative.
- Motivated, forward thinker.
- Problem solver with the ability to process information accurately and effectively, interpreting data as required.
- High levels of integrity and loyalty.
- Sensitive and empathetic in distressing situations as well as ability to work under pressure/in stressful situations.

Other requirements
Essential
- Enhanced Disclosure Barring Service (DBS) check.
- Occupational health clearance.
- Meet the requirements and produce evidence for revalidation.
- Evidence of continuing professional development (CPD) commensurate with the role of an ANP/UCP.
- Access to own transport and ability to travel across locality on a regular basis.
- Flexibility to work outside core office hours.
Qualifications
Essential
- Qualified healthcare professional
- Educated to Master's degree as detailed within the HEE document titled Multi-professional framework for advanced clinical practice in England.
- Qualified independent prescriber - V300
- Registered on the appropriate professional register
