An exciting opportunity has arisen for a dedicated and motivated existing Cardiac Nurse to work within the Cardiac Rehabilitation Team in Cardiac Services of Birmingham Community Healthcare NHS Foundation Trust Adults and Community Services Division. Training will be offered to meet the services requirements and ensure you are fully supported in your career development.

Main duties of the job

As a band 6 cardiac specialist nurse within the team you will be required to assess, plan, deliver and evaluate patient centred care including medication optimisation, symptom management, deliver cardiac rehabilitation programmes, and education to aid improved quality of life. You will be working in partnership with patients, carers and other health and social care and voluntary agencies to promote health and well-being.

You will be required to work multi professionally as part of the Cardiac Service, providing care for individuals requiring management of their heart failure and cardiac rehabilitation. You will be required to support patients within their own homes, community clinics, cardiac venues, and virtual appointments.

Job responsibilities

You will work autonomously and have the ability to work under pressure. Excellent communication skills are essential as you will need to be committed to sharing good practice, development of robust professional networks and support your colleagues. You will have good computer skills, and are able to utilise excellent time management and organisational skills. Full training will be given to the successful candidate.

The successful applicant will be required to work Monday to Friday between the hours of 08.00 am and 16:00pm. Consideration will be given to flexible working hours that meet the needs of the service.
