If you are an experience healthcare assistant, looking for a flexible and rewarding role, this might be the perfect position for you. You will work autonomously, predominantly in the patients home, but you will also be an integral member of the wider, multi-disciplinary team.

Main duties of the job

he post holder will provide direct personal nursing care, primarily in the patients home and on occasions in other settings such as Community Hospitals, Residential and Nursing homes and Eden Valley Hospice.

The post holder will provide practical nursing care, emotional, social and psychological support to individuals with palliative care needs, their families and carers. This role involves regular and extended involvement with the stress of caring for dying patients and their families.

Job responsibilities

Person Specification

Knowledge and Skills

Essential

  • Essential criteria
  • Knowledge of palliative care issues An understanding of multi-disciplinary working and working with other organisations e.g. District Nurse Teams. Enhanced communication skills to work with patients, families and health/social care professionals. Ability to work in a complex environment, often in isolation and demonstrate effective coping mechanisms. Quality focused with an innovative approach to solving complex problems. Diplomacy Sensitive to the needs of those affected by life-limiting illness. Flexibility to work various shifts patterns over a 24 hour period 7 days a week. Ability to respond to changing work shifts. Must be able to travel autonomously across the geographical area covered by the service.

Desirable

  • IT Skills-knowledge of patient information systems e.g. EMIS

Qualifications

Essential

  • NVQ Level 2/3 or equivalent Health & Social Care (Consideration for Trainee development post for those without NVQ) Care Certificate (or willingness to work towards)

Experience

Essential

  • Significant health or social care experience

Desirable

  • Experience in Palliative Care Community experience Experience of working in isolation
