Job summary

We are looking for a dynamic and hardworking individual to join a busy Dermatology Department.

The primary purpose of the Cancer Pathway coordinator role is a central point of contact for patients on a suspicious of cancer pathway'; helping to ensure the individual needs of the patient are supported and that care is built around those needs; as well as facilitating seamless, coordinated pathway delivery including through escalation; and a better, more personalised experience for patients.

Main duties of the job

The post-holder will work within Dermatology but also closely with the wider cancer team (both clinical and administrative), as well as other key health and social care providers, to enable person centred care and efficient implementation of the pathway (including achievement of the 28 Day Faster Diagnosis Standard).

The Cancer Pathway Coordinator will be involved from the beginning of the pathway and up to the point of definitive diagnosis (whether cancer or not) tracing patients through the department. They will maintain effective communication both with and on behalf of the liaising with staff and services to ensure diagnostic tests and skin surgery are scheduled in a timely manner. It is envisaged that the Pathway coordinator will have a specific focus on ensuring any potential delays within Dermatology are overcome to enable to patient journey to be as quick and smooth as possible. The post holder will identify and resolve barriers to care, working flexibly and responsively to unlock bottlenecks and delays.

Knowledge and Experience

Essential

  • Minimum of 1 year experience, working in a hospital setting as a Healthcare Assistant or equivalent e.g. Therapy Assistant
  • In date statutory essential training
  • Experience within a dermatology setting
  • Breaking bad news course
  • Experience of undertaking holistic needs assessments

Desirable

  • Experience of working in a multi-disciplinary team in health/social care setting
  • Previous experience in any of the following areas: Cancer Services
  • Evidence of involvement in service improvement or specific areas of interest/expertise e.g. link person/champion for area
  • Certification in Breaking bad news
  • Experience in undertaking holistic needs assessment

Qualifications/Training

  • Minimum of C grade GCSE or equivalent English and Maths
