Job summary

The post holder will manage autonomously as part of a highly motivated team of Case Managers providing a high standard of care, utilising holistic skills in the assessment, planning and implementing pro-active care needs to a population with Long Term Conditions in the community settings.

The post holder will work in collaboration with Primary and Secondary care clinicians, social care and mental health colleagues, to ensure care is patient centred and appropriate to their assessed needs.

The post holder will be a champion for Manchester Case Management and be able to demonstrate their clinical and professional skills and competence in team work, communication, delegation, care planning / co-ordination of individual's needs, medicine management, health and case management. The role may on occasion require the post holder to travel between the 4 neighbourhood sites and to patients homes.

Main duties of the job

Manchester Case Management (MCM) is an exciting new service being developed in Manchester, building on the successful High Impact Primary Care (HIPC) pilots and Active Case Management services.

We are looking for an experienced nurse to join our friendly supportive team. You will experience a wide range of patients journeys and work closely with your colleagues to promote health education, well-being and plan care to improve patients lives and health across Central Manchester

Working across the Central Manchester Locality the Case Manager will undertake the care of a defined caseload of patients working closely with and based within one of our four integrated neighbourhood teams .You will have the relevant experience and skills of a case manager to undertake this role.

Person Specification

Qualifications

Essential

  • Registration with NMC
  • Recognised teaching/mentor qualification

Desirable

  • Non medical prescriber V300
  • Documented evidence of having undertaken relevant further study or training appropriate to the role of Case Manager
  • Degree/diploma in Clinical Nursing or evidence of working towards
  • Leadership/management qualification

Experience

Essential

  • Significant post registration experience
  • Demonstrate a working knowledge of community nursing, case management and management of a patient within a community setting

Desirable

  • Experience of working within other relevant areas of healthcare

  • Excellent communication skills (written & verbal)
