Job summary
We are looking for a motivated, dynamic and positive band 6 nurse with excellent communication and record keeping skills to work within the care coordination team in all Claire House settings; Wirral Hospice, Liverpool Hospice and the community setting. The individual will be caring, professional and capable of working across teams and services internally and externally.
We are looking for extensive experience and knowledge within nursing care, to include effective leadership skills and with a knowledge of safeguarding procedures. The role involves supporting families holistically within their journey at Claire House, participating and chairing meetings and is the main point of contact for a large caseload of families and professionals. To build positive relationships, gain updates, work collaboratively and provide appropriate support to families in times of need.
This role is full-time and can be worked flexibly across 4 days, minimum of 30 hours is required, predominantly office hours, with an expectation to work flexibility over a 7-day period if needed, with an option to work at home at times; dependent on service need.
Key skills for this job include: Experience working with children/young people and families, excellent communication skills, excellent organisational skills, ability to lead a small team and to work autonomously. To be able to prioritise, manage time and workload, problem solve and meet deadlines. Good IT skills and a car driver is essential.

PART 1: JOB PROFILE
1. Main Purpose of Job
To be responsible for following the journey of children/young people and their families once they have been through the referral process.
To be part of a care coordination team, overseeing the total caseload ensuring that all children receive the right service, at the right time, for the right amount of time. Acting as the main point of contact for families and professionals.
To practice and promote a high level of specialist palliative care, encompassing a supportive and holistic approach to the child/young person and their families with a life-limiting condition. The post holder is responsible as a registered, accountable and autonomous nurse to act in the best interests of the child/ young person and their family at all times. You will maximise the use of the multi-skilled team to plan, deliver, evaluate and maintain safe, compassionate and effective care.
As a Registered Practitioner, you will be required to demonstrate leadership qualities; being open, transparent and trustworthy. This is in accordance with the Nursing and Midwifery Council (NMC) Code of Practice and Claire House vision and values.

Job responsibilities
Scope of Job
To act as a resource for all members of the multi-disciplinary team providing supervision, guidance, care and support
To act as a contact worker throughout the child/young persons Claire House journey demonstrating core/advanced communication skills depending on the specific needs of the child/young person and their family
To assess, plan, implement and evaluate the clinical/emotional requirements of children/young people with complex, palliative and end of life care needs
To liaise with external multidisciplinary agencies to ensure the care provided through the Hospice is coordinated and delivered to meet the specific needs of the child, young person, and family
Dimensions and Limits of Authority
To work without supervision ensuring personal accountability in accordance with the Nursing and Midwifery Council (NMC) code: Standards of conduct, performance and ethics for nurses and midwives
To report to the Team Lead of the Care Coordination Team
Qualification and Skills Level
Professional background as a Registered Nurse Child, Adult or Learning Disabilities
Mentorship qualification
Knowledge and experience in paediatric palliative care
Demonstrate clinical leadership/qualification
Wide knowledge of a range clinical skills
Extensive years post registration
Appraiser training/qualification
Responsibility for Staff
To act as a positive role model by displaying CH values, attitudes and behaviour. In clinical practice, role model by using evidence/research based care and practice
To take responsibility for the appropriate delegation of duties to other members of the care coordination team
To participate in in-service training programmes and teaching sessions
To participate in the orientation of new staff, students and volunteers. Acting as mentor/sign off mentor as and when required
To act as a clinical supervisor and participate in ongoing training
To act as an appraiser for other members of staff in the team if appropriate
Responsibility for Patient Care
Be the main point of contact through the child/young persons Claire House journey, including pre and post planned and unplanned stays and day care sessions
To manage a large caseload of children/young people, maintaining regular contact and liaising as required with key professionals to include health and social updates, as well as ongoing support
Refer to internal spoke services and monitor internal offers of care
Have an understanding of the planned and unplanned offers of care
Working in partnership with parents/carers to identify the care needs of the child/young person and plan care recognising best practice and evidence based guidelines
To liaise with therapists, learning disability nurses, social workers and specialist groups in order to produce an individualised care plan incorporating an individuals care, moving & handling and behavioural assessment plans
To be responsible for assessing, planning, implementing and evaluating care based on current evidence-based and researched best practice. Amendments will be negotiated with the child/young person and their parents/carers, documented and communicated to the other members of the team and external professionals
To have strong communication skills, able to adapt to differing needs of children, young people, and families
To ensure all children and young people receive the right care by the right service for the right amount of time
To accurately evaluate and document the care of the child/young person in accordance with NMCs guidance on documentation
To share responsibility within the Hospice in order to maintain a safe but homely environment taking into account health and safety, including infection control and risk assessments
To ensure patient safety by appropriate delegation of duties to suitably trained members of staff
To attend regular meetings with the multi-disciplinary team to discuss individual children, respecting and inviting the sharing of skills and expertise
To liaise with outside Agencies from health, education and social care involved in the care and support of children/young people and their families
To work alongside, supervise and induct new members of staff ensuring clinical tasks and work load are delegated appropriately
To be competent in Claire Houses core and extended clinical competencies
To participate and attend Multi-Disciplinary Team meetings in the community setting
