Job summary

An exciting opportunity has arisen for a Chaplain for Wellbeing in Whitby Coast and Moors Primary Care Network.

The PCN recognises the importance of pastoral and spiritual care in the provision of true holistic care. We are looking for an experienced pastoral person of faith to become part of our growing team.

The role will involve providing pastoral and spiritual care for patients in a primary care setting. To find out more information about this type of chaplaincy see below:

The successful candidate will be the first Chaplain to work in the PCN and will have the opportunity to develop a role that can evolve to meet the needs of the local population.

Main duties of the job

To provide pastoral and spiritual care for patients through one to one sessions in GP practices and in patient's homes.

To join in the work and development of a newly proposed Holistic Care Team.

To offer a listening service and other support to staff as requested.

Whitby Coast and Moors PCN comprises 4 GP practices in Whitby and the surrounding area. Our member practices are Whitby Group Practice, Esk Valley Medical Practice, Staithes Surgery and Sleights and Sandsend Medical Practice.

Job responsibilities

Provide pastoral and spiritual care interventions for patients. This will usually be provided in one-to-one sessions in GP practices or in patient's homes.

Provide listening, guidance and other support to staff as requested.

Promote health and wellbeing by providing a highly skilled listening and guidance service, working with all patients irrespective of their faith, beliefs or cultural background.

Work closely with the PCN Holistic Care Team to support the care of patients with complex needs.

Regularly attend and contribute to the Holistic Care Team multi professional meeting.

Seek out and use appropriate resources and signposting to support the wellbeing and holistic care of those referred.

Work with the PCN to develop appropriate referral pathways to the listening and guidance service, supported by relevant communication tools.

Work with the PCN to promote and support staff wellbeing initiatives.

Serve as a resource to the PCN on spiritual, ethical and religious matters and provide guidance in areas of specialist knowledge

Work in partnership with other organisations, supporting a joined-up approach to holistic care and facilitating inter-agency referrals where appropriate

Use wellbeing assessment tools to measure progress and aid evaluation.

Share evaluation and audit findings to promote learning and service development.

Promote spirituality as a quality of organisational life through personal integrity in attitude and behaviour

Undertake training in order to meet the standards set by the UK Board of Healthcare Chaplains (UKBHC)

Follow good practice, adhering to the Code of Conduct developed by the UKBHC and to practice in accordance with the Capabilities and Competencies described by this Board.

Comply with all required policies and procedures including mandatory training.

Undertake and engage with appropriate continuing professional development, supervision and appraisal in line with PCN requirements and to enable accreditation (and annual reaccreditation) with the Association of Primary Care Chaplaincy and registration with the UK Board of Healthcare Chaplaincy.

Carry out any other relevant tasks as reasonably directed.
