Gate 33 is a busy surgical ward and as the non-clinical coordinator you will be an integral part of the team, helping to streamline processes, maximise patient flow and make a positive impact on patient care. You'll need to be able to work under pressure and have excellent communication skills.

Main duties of the job

The successful applicant will be responsible for coordinating and facilitating the discharges of patients identified as medically optimised for discharge. Arranging medications for discharge, arranging transport for discharge, liaising with care providers and care homes to enable a timely discharge, ensuring good use of the discharge lounge when availability allows. Excellent communication skills are essential to ensure patient and families are kept informed about discharge plans.

The successful applicants will be responsible for keeping excellent contact with the patient flow matron, they will work closely with the ward domestics and the nursing staff to ensure bed spaces are cleaned and ready to accept admissions in a timely fashion.

You would need to be confident, an excellent communicator a mature decision maker and work well under pressure, you will be efficient and organised with an eye for detail and commitment to improve the patient experience.

Your role will involve using a range of IT systems .

Job responsibilities

Working under the direction of a registrant, participate fully in the delivery of planned nursing task to aid a safe and timely discharge, within the boundaries of your role which may include identifying bloods to be taken, cannulate patients requiring diagnostic investigations,

Ability to provide essential patient care to patients when required.

Track discharge medications supporting timely discharges.

Support discharge planning during ward board rounds.

Working with the clinical team to support in a timely and sensitive manner any referrals the patient may require.

Support to ensure reason to reside and estimated date of discharge is completed and accurate.

Book patient transport booking and troubleshoot transport issues.

Distribution and collection of patient and visitor feedback

Work closely with MDT, clinical site manager, clinical flow team and flow matron to ensure flow is maintained through the ward at all times.

To work with the nurse in charge to complete delegated tasks which ensure that patients are discharged home in a timely manner.

To work with the nursing team to ensure that all educational needs have been met prior to the patient being discharged, e.g., catheter care, stoma care, wound care advice.

To work with the nursing teams to ensure a seamless discharge between the hospital and the community is maintained. This includes but not limited to, ordering equipment e.g., catheter supplies, nephrostomy supplies, district nurse requests, outpatient appointments e.g., TWOC clinic.

To work with allied health professionals to ensure that patients are discharged home safely and in a timely manner with the correct support that is required on discharge e.g., equipment, rehabilitation, care package.

To ensure that bed spaces are fully cleaned and prepared for the next patient.

Assist the Nurse in Charge with delegated tasks, supporting with the flow of patients, including dealing with telephone enquiries contacting patients families etc. whilst maintaining confidentiality at all times.

Support in the review of stranded and super stranded patients.

Maintain accurate records of patient flow through the unit.

Demonstrates own duties to new starters and supervises work of students and HealthCare Support Workers

Frequent exposure to emotional/distressing events, e.g., breaking of bad news.

Participate in patient and public involvement activities.

Undertake mandatory training and role specific training relevant to role.

Acts on own initiative when delivering patient care with supervision accessible.

To work alongside the clinical team supporting patients on the ward to be nursed in an appropriate ward/area.

Undertakes a range of core clinical skills which may include:

Venepuncture

Patient observations including Blood pressure, Oxygen saturations, heart rate, respirations, temperature, pain score, News score

Peripheral Cannula removal

Swabbing to include MRSA, FLU, COVID,

Obtain samples including urine dip, from a catheter, MSU and faeces, sputum,

Blood glucose and ketone testing

ECG

Observe wounds and change simple dressings

Bladder scan

Skin assessment

Catheter removal

Female catheter insertion

Pregnancy testing

There may also be role specific clinical skills that you will be required to develop competency in to undertake your role (see Addendum One).

Addendum One

There may also be specialty specific clinical skills that align to the band 3 CSW role profile that you will be required to develop competency in, in order to undertake your role. These skills will be discussed with you with your line manager.

Qualifications

Essential

  • External staff must have:
  • Level 2 in numeracy and literacy (or equivalent)
  • Completed Care Certificate accredited by e-learning for healthcare evidence of both theory and all practical elements of the skills for health 15 standards (training completed via e-learning only will not be accepted.
  • Plus, one of the following:
  • Vocational Level 3 qualification in a Health-related subject regulated by OFQUAL.
  • Or any new employees who can evidence competency of core clinical skills, as outlined above in the CSW Band 3 JD
  • For existing Mid Yorkshire staff:
  • Evidence of working in a Band 3 CSW role and hold or be willing to attend the Care Certificate.
  • Evidence of completion of the Band 2 HCSW core competency framework and hold or be willing to attend the Care Certificate.
