Job summary
This role is based the West District Nurse Community Team
Location: Acomb
The role of the Clinical Coordinator is to be a point of contact for the team, handling all referrals and tasks made by our single point of access team, to triage these tasks and allocate to the appropriate staff in the team. You will require excellent communication skills as you will speak to patients, carers, GPs and other members of the multi-disciplinary team.
You will require the skills to handle a variety of conversations with patients, sometimes during times when patients feel upset or anxious. The clinical coordinator will require good knowledge of SystmOne and patient care needs. Initial training will be provided and ongoing support will be provided by the team leaders.

Main duties of the job
The successful candidate is also expected to undertake clinical duties such as wound and catheter management, administration of medications and phlebotomy.
The working pattern will be flexible within the current shift patterns, to meet the needs of the service. It is expected that this role will predominately be Monday - Friday, however there is an expectation that the successful candidate is prepared to work all shifts across the day, and 7 days per week if required.
The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy. Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required.

Person Specification
Education, Qualifications & training
Essential
- HNC or NVQ Level 3 or equivalent evidence of working at that level
- Computer literate with knowledge of IT systems such as word and excel
- Educated to GCSE/O' Level standard in core subjects or evidence of working at that level
- Evidence of continuing professional development
Experience and Knowledge
Essential
- Minimum 2 years' experience within the NHS or similar environment (e.g. social services or other public services
- Experience of working with multi professional teams
- Experience of working within a similar health and social care setting
Desirable
- Understanding of discharge planning and patient flow
- Insight into roles and responsibilities
- Knowledge of information systems e.g. CPD and Systm One
- Experience of telephone triage and patient contact
