Working Hours: 36 hours per week, Monday to Friday.

Location: Ashton GP Service, Ashton Under Lyne, OL6 7SR, Gordon Street Medical Centre, Ashton Under Lyne,OL6 6PR and Guide Bridge Medical Practice, Audenshaw, M34 5HY.

An exciting opportunity has arisen for an enthusiastic and dedicated Clinical Pharmacist to join our practice teams and provide care for our patients at Ashton GP Service, Gordon Street Medical Centre and Guide Bridge Medical Practice.

The Clinical Pharmacist will be part of a busy and proactive team working alongside clinical and non-clinical colleagues to innovate and progress the clinical pharmacy service, enhancing patient care and outcomes.

The successful candidate will participate in and support the Deputy Lead Pharmacist, medicines optimisation to deliver change for the benefit of patients and/or the organisation.

You will be a key member of the senior clinical team being proactive in providing safer prescribing, providing expertise in clinical medicines advice, while addressing both public and social care needs of patients in their care.

Main duties of the job

  • The successful applicant will be a registered pharmacist with a non-medical prescribing qualification (V300).
  • You will provide support to other clinicians with regards to prescription and medication queries/repeat prescriptions, and will demonstrate clinical reasoning and critical thinking, as well as the undertaking of comprehensive structured medication reviews and assessing/processing repeat prescription requests. The role may also involve the management of long term conditions and diagnosed or undiagnosed undifferentiated illness within your scope of practice.
  • You will ensure that services integrate with community and hospital pharmacy to help improve patient outcomes, ensure better access to healthcare and help manage workload. There will be an element of clinical audits and be expected to feedback the results and implement changes in conjunction with the practice/service as part of the medicines optimisation programme.
  • You will lead on the development of innovation bids, new services or a new mode of service delivery to meet the demands of the changing healthcare environment.
