Our Community First Team has opportunities for Community Health & Social Care Assistants in Derby City.
We support individuals discharged from acute hospitals and those requiring urgent community response. These individuals need short-term/interim support in their homes. The team is committed to assisting and supporting people at home, working on a seven-day rota.

We are not accepting applications via TRAC, to apply for one of our HCA posts, just turn up on the day and meet the team!
Please note this role requires travel in the community and public transport may not be fast enough for these visits.

Main duties of the job
What are the hours?
Various shift patterns, (Including unsocial hours, weekends, and Bank Holidays)
Minimum 12 hours/week considered, up to 36 hours/week
Where would I be working?
Base:Florence Nightingale Community Hospital/ Perth House (and travelling to people's homes)
Area Covered: Derby City and surrounding areas
This job involves traveling to different places in the community. Public transport might not be fast enough for these visits.
Summary of main duties:
Maintaining high standards in work and documentation, reflecting DCHS values.
Ensuring quality patient care through knowledge development and competency support for clinicians.
Assisting clinicians in delivering agreed treatment and care programs.
Providing individual care without direct supervision in the community.
Supporting patients to optimize their health and independence.
Offering support to Perth House in Chaddesden when required.

Must hold - GCSE Maths & English (grade C / 4 or above) or equivalent
NVQ 3 Level (or equivalent) in a relevant subject or equivalent previous proven experience
